Hi all!

Hope you are all doing well. My dad just got his scans back, and to go along with his rising PSA (over 260 now), the said they suspect he has an epidural tumour. Our MO typically glazes past his scan results as she doesn’t seem them significant, I want to make sure I am right to be worried here.

His next step was Radium-223 to help relieve some of that bone pain. Am I right to assume that would be out of the picture with an epidural tumour? Is there anything left for us to do here?

I fear him getting this news as he’s been in such a state of depression since our last appointment. Lately he hasn’t been taking his pain medicine either because he’s just too tired to sit up, or he is so shaken he drops the pills. I know his prognosis isn’t looking good and I can see the looks on our nurses faces at times because he is cachexic, muscle is completely wasted. I don’t know that I have any other questions, just a scared daughter who feels completely helpless. I wish things were better.

The epidural tumor doesn't preclude Xofigo, but may lead to spinal cord compression. To prevent that, he may want to have it removed via SBRT, surgery, or ablation. Perhaps ask for a neurological consult.
