Odd statement found in STAMPEDE trial results

Most probable they were referring to bone metastases and their complications and not to osteopenia and/or osteoporosis.

But hormone therapy doesn't cause bone metastases , does it?

Hormone therapy causes osteopenia and osteoporosis and eventually may cause fractures, a serious event.

The cancer causes bone metastases with bone destruction, fractures etc.

Abiraterone provides a better control of the cancerthan ADT alone consequently most probable less severe skeletal events than in treatment with hormone therapy. alone.

In final analysis the reduction in cancer bone adverse events due to better control of the cancer by abiraterone is associated with a reduction of the risk of serious bone problems when compare with ADT alone.

Estradiol (the active part of estrogen) will offset the miserable side effects of ADT. There is a risk of gynecomastia, and that cosmetic problem alone seems to stop a lot (most?) of men on long term ADT from taking it. So they live for months, years, in misery. Some stop ADT because they can't stand the side effects, or don't want to live with the bone fracture risk of ADT-induced osteoporosis.

I am on estradiol, have been for several years. Dr. Richard Wassersug has discussed this as well. As part of my primary therapy 10 years ago, I was on Lupron ADT and developed osteopenia in just over 18 months. I was put on Prolia until ADT ended after 3 years of primary therapy. About six months after that, we stopped Prolia. Three years later, my PSA was rising, so restarted permanent ADT. Fearing osteoporosis, I also started estradiol (patches). After 3 more years, my bone density finally recovered all the way to NORMAL. Further, I feel great, no hot flashes, good energy, memory as good as ever; aside from complete sexual dysfunction (a known side effect of long term ADT anyway), I feel better than ever. And my PSA is still undetectable.

Due to these positive results, I am a strong proponent of estradiol for those on long term ADT.
