Diagnosed w MHSPC May 2021. Gleason 5+4 . PSA 780.
Started ADT right away . Went through 2021 and 2022 H2 body adjusting to all the changes. 2023 H1 pretty solid , feeling pretty good. Recent PSA went from 0.32 to 0.64. CT unchanged but bone scan w contrast shows new uptake. RO recommending Xofigo.
Can you please share your experience on this treatment and whether you modified your ADT during the 20 weeks ? Did you have any side effects? What was the outcome +12 mos?Thanks gents and keep up the fight !!!💪

Are the uptakes in bone Mets showing in multiple spots? If only one spot, salvage radiation (external beam radiation) is in order. If multiple spots, Xofigo is called for.
I went though a Xofigo treatment with no side effects. But everyone responds differently.
There are a couple of spots identified as bone mets in the scan. RO has downplayed use of radiation in my case . Did the use of Xofigo defer the use of more aggressive treatments in your case? Did it do the job?
completed six cycles of Xofigo earlier this year. Some bone pain and an episode of diarrhea each cycle. They will want you on a bone strengthening agent Zometa for me. Honestly the Zometa caused more bone pain than the Xofigo. Was on ADT - Eligard the whole time. Mixed results - existing Mets got smaller but new Mets appeared. PSA rose, ALP dropped. Went straight from Xofigo to Docetaxel a week apart. Had issues with low hemoglobin which messed with Docetaxel schedule. Xofigo was worth it to me.
My husband's experience with Xofigo was very similar, maintain his ADT throughout. He had minimal noticable side effects, and the results were mixed - but we think he is doing better than he would have without it. Also moving on to docetaxel.
He was on lupron and enzalutamide for the duration, but both had failed to fully contain the PSA rise. Xofigo does not directly impact PSA value. Hi rose slowly, jumped after completion of the treatments.
Thank you for replying. I had 6 sessions of docetaxel and it did get much better gradually in a couple of months after #6. So was Xofigo easier on him than Docetaxel?
