Just a short background. Surgery in 2009, IMRT completed in 08/2021 when PSA was at 0.12. I had a negative PSMA pet scan when my PSA was 0.3. My latest PSA result on 11/4/20222 was 0.42 up from 0.3 6 months ago. Because of my age, my Oncologist wants to wait as long as possible before starting me on ADT. I'm meeting with him in 3 days and wondering if he will change his mind about starting ADT. Your opinions are appreciated.

I was also negative on PSMA Pet scan with 0.3. We waited six months, then did another PSMA Pet scan. At this time three lymph nodes showed low SUV peak values. We re-startet ADT plus RT. Now my PSA is undetectable again.
Your PSADT is too slow (about a year) to start ADT yet. If it gets to less than 6 months, you can revisit.
I would* take Bicalutamide to bring PSA down to 0.05 and at that point would adjust the dose to stabilize it at this level.
(*) Actually what I have been doing for a year now.
