I have been on Xtandi/Eligard now for about 5 months. At a psa of around 1.9 last March psma pet picked up 3 nodules in my lungs. All less than a cm. I am hormone sensitive and my current psa is undetectable.
This July I had a ct scan of chest/abdomen/pelvis. The mets in the lung have decreased in size, which is great, but they picked up a 3cm mass on my left kidney. A followup scan indicates renal cell carcinoma. Likely stage 1. Easily resectable and I do not lose the entire kidney.
I have been walking 10,000 steps a day or more everyday for a long time (about two years now). Everytime i get a little foggy/tired from the hormones I move and it feels better.
So my concern with this surgery is how fast i will recover while on hormones. My next eligard shot coincidentally is the week after surgery.
Do I take it or wait a month? Do i stop xtandi for a week or two whilst i heal from the partial nephrectomy?
Anyone with similar experience? I would think that the two drs would coordinate (same hospital) but they seemingly don’t.
I don’t think that you stop either drug. But ask your doctors, and talk to them about coordinating with one another. Back in 2015 I had extensive Mohs surgery to remove melanoma while I was receiving chemo per CHAARTED and taking Xtandi and Lupron. My MO and surgeon stayed in communication since there was a danger of the chemo affecting my WBC counts. I think if you speak with both about communication they’ll come around otherwise you’ll have to be the intermediary.
I had hip replacement in July and my MO suggested to stop Xtandi a day or two prior to surgery because of unknown interaction, mostly for anaesthesia drugs. A reasonable request and the anaesthesiologist made no comment about any drug interactions so I suspended for two days and all went well.
Good morning, I am on Lupron and Xtandi, I had partial knee replacement in May 2023. I needed to get clearance from my PCP, and my Cardiogist, they only said to ask my MO for his recommendation for taking my meds. I was in the middle of a 3 month cycle for my Lupron, no problems there. The Xtandi, they said I could stop it for like 2 weeks, if I wanted to, but it was up to me. I decided to do a 1/2 dose for a week and then went back to full dose, 160mg. All went well and I must say, I had very little brain fog the week after my surgery. This is just my experience and what I did, ask your dr and listen to him. If you cannot get the two drs to talk to each other, be the advocate between them. I am not a Dr just another man along this journey trying to make the best of this cancer. All the best to you.

So this is how it went for those interested in history at this point
1) I stayed on xtandi the entire time except for the one overnight in the hospital
2) Two weeks out and i am almost back to my normal self. I have gotten back up to 10000 steps/day three times in the last 5 days. Have to do the steps in increments. I normally walk 5000 steps right away in the am, but i found if i did that i was tired all day like I had walked 20000 steps. So 1000 steps per hour gets it done.
