Hi - I've been on Xtandi for the past year which also caused incredible fatigue for me... The only remedy that works for me is regular strength training exercise. It helps incredibly...
As for the insomnia, also a symptom I got from Xtandi, I get total relief with a piece of full spectrum (including THC) cannabis oil infused chocolate that I make myself before bedtime... Many brothers on this forum use cannabis gummies from dispensaries with good results...
Thanks @GSDF. I have a walking regimen I've been trying to keep up, but that just wipes me out now. Any advice on strength training that doesn't require expensive equipment or a gym membership?

I too take Xtandi and experience deep fatigue as well. Different than you tho, the tiredness and fatigue make me sleepy and I sleep all the time, seems like. I frequently have to battle to stay awake.
You might ask your oncologist or pcp for a trial prescription of Xanax to see if that helps you get to sleep better.
If you live in a recreational marijuana state, call up one of your local driveway delivery services and ask what product they have that promotes sleep. Delicious gummies are kind of taking over smoking cannabis , for people worried about smoking byproducts.
If you live in a state where marijuana is still illegal, the federal government differentiates between marijuana and hemp as being two different plants. Hemp is federally legal nationwide , but certain states might have local restrictions on hemp products.
Hemp wise , it’s commonly / easily available in local cbd stores or via mail order.
Thc delta9, marijuana gummies make you high and sleepy for most people. It’s marijuana in gummie form and numerous types and flavors exist.
Delta 8 gummies are laugh factories but make you very sleepy a while after ingestion.
Gummies with thc-p don’t make you all that high, but alleviate pain in many people and eventually make you really sleepy. ThcP is long lasting and can last for a couple of days in some people.
Gummies with thcV in them is useful ( in many people ) for alertness and energy stimulation. Still after a while they also put most people to sleep. They don’t have much ( if any ) of a “ high “ factor.
Of course, thc derivatives don’t work the same in everyone, everyone is different …. and , as always, it’s best to discuss this with your medical team before use.
I don’t want to hawk brands on the group, pm me if you want the name of a known reliable company for mail order. Gimmes like quality marijuana are not inexpensive .

I hear you brother... When I started Xtandi I saw posts on this forum that to diminish the fatigue was workouts with strength training (weights)... So I joined Anytime Fitness at $50.00/mo... I found it not only expensive but tedious to drive to the gym from my rural town...
So I bought two hand held 10lb barbells and this apparatus from Amazon and I can now do strength training exercises every bit as effective as the gym at home:

As for the cannabis, for me, the THC is crucial. CBD oil in itself helps many but it never did me... So I use the full spectrum cannabis oil infused chocolate...
Other fellows on this forum, like Kaliber, may chime in with recommendations on dispensary edibles like gummies that they find very effective... and what Magnus says above is also helpful with the melatonin... I take 80 mg (four 20 mg tabs) an hour before bed with the cannabis... I sleep like a baby 8 straight hours through... 🙂

Sounds grim, sorry. Well like others I get extreme fatigue but fortunately have no difficulty sleeping. Often in bed, latest at 21.30 and can be there 12 hours with the odd visit to the loo.
I'm not on Xtandi but firmagon does a good job of waking me up several times a night with hot flashes, so while I am generally sleeping through the night, I was getting up 4 - 5 times and would be awake 5 - 10 minutes each time with the occasional difficulty going back to sleep.
I've added two things to my night time ritual that, at least for 3 nights now, have greatly improved my sleep. First, I started taking Gabapentin at bedtime and it ended my nighttime hot flashes. And second, from reading about it on this board, I ordered delta-8, 25mg gummies, cut them in half, and take one as well. I'm getting up max 2 times per night to pee but go immediately back to sleep. I cut them in half because the first night, when I took the entire gummie at bedtime, when I woke up, I walked to the bathroom like I had just finished a 6 pack. Would have been fun if taken earlier in the night for shits and giggles, but not necessary to have a full dose when going to bed for the sleep benefits.
Has not happened to me with Erleada and Lupron; I'd suggest those are not the cause. If nothing else do a sleep study.
Thanks for all the advice. My MO suggested taking Xtandi at night just before bed. This has helped with the insomnia. I'm still weak as a feather, but at least I can sleep through the night. Could probably sleep all day too.
Turns out the fatigue was probably due to covid. I got extremely tired after a 1/2 mile walk. Blood oxygen tested at 77 percent at home. Was admitted to hospital that evening. Being treated for Covid (remdesivir and deximethizone) and probable pneumonia (multiple antibiotics).
