I'm not quite understanding the timing of dosing of Xtandi during BAT. Dr Denmeade wrote this article ( and i think it's great) but I'm not getting when you take Xtandi during BAT. The BAT cycle is 28 days. Do you take Xtandi during the cycle or is it when BAT starts to fail, then take it and start BAT again?

BAT and Xtandi
In his current trials with BAT and Xtandi, patients (who are asymptomatic mCRPC and who have failed Zytiga) stay on ADT throughout. In addition, they are randomly assigned to one of 3 groups:
A. Xtandi
B. Two months of high T followed by two months of Xtandi. Repeat.
C. Up to four months of high T, if progression, followed by Xtandi. If progression, T is repeated.

Patients are very closely monitored with PSA and scans.
Sartor discontinues Xtandi during BAT which makes sense since you don’t want to block the tastosterone from reaching cancer cells. He has you remain on Lupron so the testosterone levels fall quickly hence the cycling or bipolar. Eventually BATis no longer effective and you give Xtandi another try, often it reactivates Xtandi thus extending its usefulness.

The third group in the study is to use testosterone until it fails and go on Xtandi. When it fails go back to testosterone. By “fails” I mean a PSA rise to a certain level
