Neuroendocrine PCa

Hi, My father was diagnosed with neuroendocrine in the liver mets. After 6 rounds of doxitacel, enzulamite and degarelix.

The doctor has said chemo + immunotherapy. And given us 9-12 months. Is that really it ?

Could you try Dr Sen to see if Fapi or PRRT would be any help?

Last time I checked with Dr Ishita Sen she was on top of the subject , lu- DTPA-SC16 for T-NEPC. Sooner or later they will offer this treatment. MSKCC had a working version 18 months ago, they should be about to start a trial.

It is free and they work with your oncologist. I don't know if it is a waste of time but might be worth a look.

Other than those the only thing I can think of is to travel to trials in US but really expensive. Have they taken a biopsy to see if Keytruda or olaparib would work ?

Thank you for your inputs. They have not suggested a biopsy but have done the IHC on the Fnac

His IHC clinical details: Clinical details - K/C/O Ca prostate.

SOURCE : Liver SOL FNAC cell block.

GROSS DESCRIPTION :

Received single paraffin block and single H&E stained slide labelled

as 4556/23.

MICROSCOPIC DESCRIPTION :

Received cell block sections show a few discohesive clusters of

atypical round small to medium sized cells.

Areas of necrosis noted.

Immunohistochemistry :

-----------------------

LAB NO.: IH - 820/2023

CK (AE1+AE3) -------------------- : Positive.

INSM-1 ( RBT-INSM1 )------------- : Positive.

PSA (QR038)---------------------- : Negative.

NKX3.1 (BSH3.2) ----------------- : Negative.

IMPRESSION : Metastatic small cell neuroendocrine carcinoma.

If they have enough tissue left over, it may be useful to do a somatostatin IHC. It may give nothing useful, but there is a very small chance that it might show that the tumor is vulnerable to Lutathera.
