Rising PSA after SBRT

3 weeks ago I had 5 rounds of SBRT after a PSMA scan showed a spot suspicious of bone met. Yesterday, I went for my blood test...my PSA had risen to 14.9 from 8.5 in March.I had expected the PSA to decline after SBRT. What does the increase in PSA mean? That the rib met was a false positive?

What are the recommended treatments now going forward? I'm meeting my oncologist tomorrow...what questions should I ask him?

Thanks for any advice.

It means dead cancer cells dump a lot of PSA into the bloodstream. Test again in 2 months.

So I need not be unduly concerned by the rise? Thanks for your reply Allen.Should I ask for ADT to start immediately or wait for the PSA test then decide?

I keep reading this:

Surprisingly, research suggests that radiation patients who go through a bounce actually have better long-term outcomes than those who don’t.

That's not a bounce. Bounces refer to prostate treatment.

The article that you cite refers to treatment of the prostate, not treatment of metastases as Tall_Allen has noted.

you only read the yellow paragraph. Which was explanatory

if you kept reading

Our case suggests that this effect can also be seen in patients who have undergone previous radical prostatectomy and PSA bounce should be considered in patients with a PSA rise following salvage radiotherapy.

Salvage radiotherapy and/or previous RALP still are not the same as radiation to distant metastases.

You should never be off ADT, unless you need a break.
