Seeking advice for father. RP in 2014 with clear margins and periprostatic node positive. After 2 years psa jumped to 2. A solitary met in femur. Sbrt given plus ADT started in feb 2017. Psa remained <.008 on ADT. Since nov 2019 psa started increasing till .09 when pana showed a solitary met on humerus. T was 19. We have been advised sbrt To humerus plus zytiga. Post 4 week sbrt psa is .05. Any suggestions if sbrt was effective and if we can delay zytiga or start it right away

Of course, start Zytiga ASAP. All you have accomplished by zapping the bone met is treating PSA - that is not the same as treating the cancer.

Is there benefit in delaying zytiga - for instance would it give a prolonged response down the line?

The opposite - it has the greatest effect when used earlier, diminishing over time. That seems to be true of almost all prostate cancer medications.
