I need your experience, your help.

I m here for my father, i m here to find some answers and hope.

My father found out in January 2022 (after his leg hurt about 2 months and nobody knew what he had) he s got PC with 120 PSA and some bone mets. When he had his CT, doctors found some modifications on lungs and pleura so...he had 6 sessions of docetaxel and prednison and eligard. Al the modifications in lungs and pleura disappeared and bone mets lowered in dimensions. After he finished docetaxel sessions he started Xgeva too.

Unfortunately, in november his PSA started rising, and grew in aprox three weeks from 0,3 to 1,69. Strangely, his testosterone level was dowm 0,3. We were devastated, this situation scared us, since then we had no peace. His doctor immediately changed his treatment and completed Eligard and Xgeva with Xtady.

Last month he had a scintigraphy and, although some stuff were seen the doctor decided that he can not treat them like mets. Such good news !!! I think his mets aren t actually as active as they were.

Tomorow he will test his PSA (3 months evaluation), the first one after including Xtady. I am so scared because of his PSA doubling time. Our doctor... what can I say, helped us right away, she didn t scared us. Also my father is feeling fine, but i am still soo scared.

My father knows nothig about his stage of disease. He s fine, he optimist and we decided not to tell him about mets and stuff. I know him and he would be devastated. Please, tell me if u know, can Xtady help him even though his PSA doulbling time was so high? Is Xtady capable to manage the situation and let us live our lives in peace?

Xtandi is very effective, it will lower the PSA value. But it will not work forever, next year he may become resistant to this drug.

I suggest to get radiation to kill most of the cancer cells. Drugs will be more effective then:

Thank you for your answer. His testosteron is low, it s 0,3, ... at least I guess is low, but PSA started rising. That was my problem.

Xtandi does not lower testosterone, it blocks the androgen receptors of the tumor cells so they cannot get testosterone. This can cause the testosterone level to increase a bit because no testosteron is consumed by the tumor cells any more.

Does Xtandi lower actual serum testosterone like Lupron-type drugs?

My understanding is trust Xtandi is an androgen-receptor blocker that works by blocking testosterone receptors and preventing testosterone from attaching to these receptors found in prostate cells.

Sure, the cancer cells might see lower testosterone from their perspective, but what about the rest of the body?

You take Xtandi in combination with Lupron. Lupron lowers the testosterone level. If you then add Xtandi to Lupron the testosterone level can increase a bit as the androgen-receptors are blocked. I do not know how blocking the androgen-receptors influences the non-tumor androgen-receptors, e.g. in the brain.

Xtandi is a great drug. If he only had one bone met, he should also have radiation to his prostate. This is called "debulking." It has been shown to increase survival in men with a small number of metastases.

We live în Sibiu but he takes treatment în Cluj. Cluj îs maybe the Best în C treatment. Thanks for your answer, he s got a large met on the sacral zone and a few online his ribs.

HEllo, again. My father had his PSA today and it s 7,15. On the 5th of January PSA was 10,87.

Do you think PSA dropped enought? I am so scared, I hoped it would be under 1.

My brother was in Xtandi for almost a year. His PSA started to double. He then went on Carboplatin/Jevanta chemotherapy.

That failed after 8 sessions, he’s now waiting for Pluvicto.
