last week. Between my 1st and 2nd infusion my PSA went from 5.3 to 9.8. my ONC said not to worry as it takes a few infusions before the PSA turns down. Between my 3rd and 4th infusion my PSA went to 11.4. Being an optimist I read this as the curve being put in as the PSA did not double like the previous reading. Optimism aside I am worried what my 4th reading might show. My Onc is also worried that she ordered a bone and Catscan. Has anyone out there been thru 4 or 5 or more before results ? Or
Was Doxetoxal abandoned for another drug? Your experience s will be appreciated.

Jevtana (cabazitaxel) is indicated for men in whom docetaxel doesn't work. Results may be further improved by adding carboplatin. Share this and discuss with your oncologist:
Yes-PSA rose on docetaxal. MO discontinued chemo after the completion of 5 cycles because of neuropathy. DH has started Pluvicto and had 30% drop in PSA after first treatment. Our best to you.
Same w DH here except PSA fell 90% on 6 Docetaxol treatments (ended up in wheelchair because of neuropathy) but rocketed immediately afterwards. Going for 4th Pluvicto treatment in 10 days. Thus far, 70% drop in PSA and pretty much no side effects beyond easily managed bone pain from day 3 to day 7. Hang on in there.
Your PSA numbers are not too high but do look at your symptoms and the scans before working out whether to continue with the chemo. I am not sure there is much to be gained by continuing chemo if it is not brining in any benefit. My husband started chemo with a PSA of 99 (having become resistant to Enzalutamide). Each Docetaxel infusion led to almost doubling of the PSA. Despite being clear that it was not working they did insist on 4 infusions after which the scans showed progression. Then switch was made to Cabazitaxel but being in the UK they would not add Carboplatin. Two infusions later, it was clear that Cabazitaxel was not working either. My husband has since had some targeted radiation for pain control but we are trying Abiraterone as it was never given before. The current indications are that it is not effective as the PSA is quite high. We measure ALP and LDH, the latter has always correlated with rising PSA and disease progression, ALP not so much.
to all off you that responded to my request for advice ,thank you kindly. I am now more understanding of my treatments early failures ,and future possible positive results if i add a few more infusions. I also can now meet with my ONC and plot future treatments with a different drug of which you have provided me names.

My spouse had four rounds of docetaxel. (Became castrate resistant with firmagon and zitiga and prednisone, after only 4-5 months.) PSA rose after each infusion of docetaxel, with a big jump before and after the 4th round. PSA just before the start of docetaxel was 18.19; in three week intervals it rose to 21.73, 26.84, 43.26, 64.50. MO ordered PSMA scan and staging scans, also changed to cabazitaxel. MO wanted him to go to pluvicto ASAP but didn't want to have a gap in treatment while waiting for all the pluvicto details to get worked out. Three weeks after cabazitaxel his PSA was 114.80. Had first Pluvicto treatment one week ago. We are very anxious to see some PSA drop! A continuous series of big jumps in PSA is hard. And chemo was hard. Our big question is whether we might see a drop after one pluvicto treatment.
