Maybe One of You Can Benefit From This: Radiation Again After 10 Years of ADT.

Basically, I have been on Lupron and Zytiga for 10 years, except for 3 vacations from both during my first 5 years. I also had my prostate removed 6 months after dx, which was "experimental" and had the largest tumor in my left acetabulum (hip) radiated 2 yrs after dx.

I started to see a rise in my PSA over the last year along with an increase in the PSA doubling time and thought I would have to shift to a new treatment. PSA had gone from: .06, .07, .10, .22, to .36. Thankfully, I was wrong- at least for the moment.

The PSMA scan indicated to my MO (Rathkoph at MSKCC in NYC) that the only active tumor (of the 4-5 that I have) that "lit up" was the one in my left hip which had been radiated 8 yrs ago. She recommended with radiate again, (this time with the more advanced type of radiation,) and see what happens. First set of labs, about 4 weeks thereafter showed a drop in PSA from .36 to .05. And, this was accompanied by a 1-2 week delay beyond the 3 month schedule for receiving my Lupron shot on time, because I contracted Covid.

I will have labs again in about 2 weeks and remain cautiously optimistic that the total impact of the radiation had not taken effect when the 1st labs were done, and also, that the recent Lupron shot will also help, and hopefully my PSA will drop further, or, at least not increase.

I will let you know.

Couldn't they biopsy your met in order to genetically analize it for further treatment?

I know that it is not easy to biopsy a bone met.

Did they mention that possibility? I believe it would be interesting to know the genetic results of the met. Maybe you have more of them but not yet visible on the PET scans.

Congratulations! I think Dana Rathkopf is excellent! I often quote her ASIS trial (combining Erleada and Zytiga), which I think didn't get the attention it deserved.

are you sure that the symbol for "less than" wasn't in front of the .05? I just got my latest draw at MSKCC and they are now using below .05 to indicate non-detect.

That is the same tactic that Dr. Sartor has recommended for me once my PSA rises to the level where a PSMA scan would be effective. Glad it had a good result for you thanks for posting.

Thanks for posting. I just started Zytiga July 1st and am still on Lupron. After 20 months of Erleada and Lupron my PSA climbed all the way up to 20, they then added Xofigo, but I was only on that for 3 months as my PSA rose to 33. So they scrapped both the Erleada and Xofigo and so far the Zytiga has brought the PSA down to 24. I'm sure it was higher than 33 when the made the change as I was off the Xofigo and Erleada for almost a month before taking my first dose of Zytiga.
