Last ct scan was 6 months ago it was clear but psa was on the rise. Over the next few months psa rose to 7.2 and then back to 6.6 6 weeks ago. My father recently had a pace maker fitted and chest X-ray revealed a couple of spots. Was sent for ct scan getting results this week. Hx diagnosed 2018 with extensive Mets responded well to chemo, adt and then recently xtandi for 2 years. I’m scared these spots suggest neuroendocrine variant. Thoughts?

If it responded to chemo, ADT and Xtandi, it definitely is not neuroendocrine. There are many types of low PSA PCa that are not NEPC.

How involved is getting the Chromogranin A blood test?
Go online to ultalabtests.com and order one yourself unless you want to go through the doctor. $168. Just to clear out my FSA upon retirement I did that for fibrinogen, ldh, chromagranin-A, hepatic function panel, bone-specific ALP, sed rate, C-reactive protein, Thyroid panels, and of course, PSA ultra sensitive and testosterone free and total. Got out of all that for around $500. My Chromagranin-A was 360 ng/ml with normal at <311, but none of the others were out of kilter so i chalked that up to me being an inflammatory kind of guy. Plus this may have been Ultalab's version of the Chromagranin-A; there seems to be more than one test.
