Has anyone heard of clinical trial NCt05125016

We had visit with oncologist,not good at all. His scrotum has been swollen think it is lympadema from the cancer. PSA over 288!! Alp ok. Doctor said only thing left is chemo, which he gives at most 40% chance of helping or above trial. This is phase 1, with drug, REGN43336 and Cemiplimab for this he needs to be in the hospital 3 days for the first 3 doses, then once weekly every 3 weeks. This sounds very scary they are checking for dose escalation and expansion. He has tried 2 prior clinical trials Keytruda, failed, the last one stabilized mets which now are expanding. He is very confused and we are both frightened as he has been feeling well fighting this for 11 years. thanks judym

It's a BITE therapy. The drug binds to two antigens on the cancer cell, and creates a binding site for T cells to attach and sttack the cancer. BITE and CAR -T therapies have strong Cytokine Release Syndrome (CRS) reactions, which can lead to the immune system attacking healthy tissue. Patients are kept in hospital after treatment, so the reaction can be managed.

It is an immunological trial, phase 1 (there is not previous clinical experience with this medication,REGN4336 ).

I'll go for chemo which is a treatment that has been shown to prolong survival. You could discuss adding a platinum compungid to the chemo (cisplatin or similar).

If he has bone mets and never had Xofigo it may be other possible treatment which could be done along with chemo. Other possibility is the clinical trials for Lu 177 PSMA.

It's called a BiTE therapy. One side of the molecule hooks onto the PSMA on the surface of the cancer cell. The other side hooks onto CD3 ( a protein that activates T cells to kill that cancer cell). Cemiplimab (Libtayo) keeps the immune reaction going - otherwise the T cells might come to recognize PSMA as part of normal cells and prevent the T-cells from killing it. There is a lot of interest in BiTE therapy now - we all hope it works.
