Is the pain in the bones or in the joints.? It is unusually that osteopenia (T value of dexa up to -2.5) or osteoporosis (T values greater than -2.5) alone cause pain..
If the dexa show osteoporosis he could be treated with denosumab or zoledronic acid infusions.
If there are bone metastases they could be causing the pain and you could discuss if treatment with denosumab or zoledronic acid is indicated in his case and if treatment to the mets with radiation may be an option to consider.
