I was a gleason 9 with lymph node involvement. No distant mets. Did HDBrachy with 25 sessions of IMRT.
I was DX 2 1/2 years ago. This is my first vacation . I'm off all drugs.
I've been on Firmagon for 2 1/2 years.
Stopped 35 days ago.
Small PSA rise and T went up considerably.
I still have my prostate.
Thoughts welcome
PSA will tend to rise as T increases through the hypogonadal range. Morgantaler's androgen receptor saturation point is around 250 ng/dL. You have to get above that to see what the PSA trend is. Don't read too much into early numbers.

So it could be a number of months before PSA rates are meaningful, no? At what PSA level should one get back on ADT?
Why do you call it a "vacation"? Isn't adjuvant ADT over, hopefully?

Pretty quick Testosterone recovery. Bet it feels good. Seems like with just radiation you should be happy with anything that low even if it appears like a modest increase

My plan was to keep measuring the PSA and T levels every 30 days
Once my PSA got above 1.5 - 2 get a PSMA scan to get better insight

I think that's right and enjoy life now. What will be will be but you feel good now and it may last for many years.

Get back on the adt pony
Keep tracking T and PSA and see how things look when your T reaches the normal range. If PSA rises above 0.5-0.7, investigate a PSMA scan. Best wishes.
The end of my adt treatment is still 18 months away but when I asked my RO about psa rising once adt is over, he said that there is no follow up treatment required until the psa reaches at least 2.
