On Cabazitaxel, PSA started increasing......PSA->15 (1 Feb 21)->13.64 (#post 3 cycle, 5 April)->15.3 (#6 cycle, 14 June)

We are currently on Cabazitaxel (post Abireterone->Docetaxel(10 cycle)).

completed 6 cycles, PSA started increasing however Dr. suggested to complete 10 cycles as disease seems stable :

PSA->15(1 Feb 21)->13.64 (#post 3 cycle 5 April)->15.3 (#6 cycle 14 June).

1. Is this sign of progression or a normal fluctuation?

2. Can PSA be decreased further?

Dr. did not suggest for pet scan now, planned to re-assess after 10 cycles.

Stable is better than rising. There may be a benefit in adding carboplatin to the next infusion:

Do u feel the disease is stable? I mean, I m a little worried to see an increase PSA from 13.64 to 15.33 b/w 3rd to 6th cycles.

Should not we go for PET CT SCAN?

That's stable. What do you think a PET/CT scan can tell you? You should at some point repeat whatever your baseline scan was.

1. Looking at your PSA trend, it seems to be fluctuating in a narrow range, and your treating doctor seems to be right in calling is as stable disease. It will be advisable to do the PSMA PET CT scan after the completion of chemotherapy unless there is significant rise in PSA levels. Hope for the best.

2. Yes your PSA levels can reduced further depending upon the response to chemotherapy.
