This MRI results are showing bone metastases in pelvic bones, femur and associated joint inflammation. This indicates that prostate cancer has spread widely into bones if he has high PSA and High ALP. No further explanation can be given without more details of his blood tests, symptoms etc. Might need urgent treatment to control further spread.
