Chemo and rising PSA

So my father has had one full treatment of chemo (Docetaxel) and went in for his second treatment yesterday. His PSA results are in and are not good still rising. He went from 373 to now 558. He also had his Lupron shot given to him yesterday as well. Dr. Wants to check psa when he comes back on for the 3rd round. Does this sound like a good plan? Is it even beneficial to receive a 3rd round if it’s not working? We are also still waiting on his genetic testing. TIA

Also his pain has not gotten worse if anything he seems a little better but it is being managed with pain meds. The Lupron shot increased his pain in his knees but that is where he has severe arthritis. It didn’t seem to knock him on his butt since he went to work today.

look at the bright side...your fathers rise in PSA could be from dying cancer cells giving off PSA from his first Docetaxel infusion. It's been known to happen.

I have the same issue, not sure yet if there’s progress with chemo until bone scan..after 5th/7 treatments. Hang in there!

So you psa has continued to rise as well? I don’t think there has been progression in bone mets I believe they have stayed the same but I think having another ct scan next visit

Yes. I think the imaging is very important. My doc said, be patient it takes a few treatments before we will see progress..it doesn’t happen overnight lol. At least he has humor about it, easy for him to say. But yes he really looks at my alkaline phosphotase. He said that number will go up before going down in my case at least.

Dying cells increase the PSA. If you are very concern and think chemo should be stopped, discuss having imaging studies and see if there is progression of the disease.

Agree. Bone scan is more definitive, but give a little time to let chemo work.

Thanks weezer! Good job looking out for pops .unfortunately he must go day by day right now . Just Getting through each round is an accomplishment . I’m waiting to read others views . Hang in there with him 💪

He seemingly did well with the first round and I believe I said in this group he seems in less pain, is moving around more and complaining less (my mother would probably disagree with that) 😂

It sometimes takes a couple of doses to have a measurable effect. Shrinkage on a CT/bone scan is a better indictor than PSA. If no response, sometimes adding carboplatin helps. Very good idea to biopsy a metastasis.

I hope that is the case and this starts to help. They will biopsy a piece of the metastasis from his bones? He has femurs,spine, pelvis and ribs I think 🤔

Agree with the others about imaging being the most important thing.

If there is progression he can also switch to second-line chemotherapy Cabazitaxel. Many have said the side effects are easier with Cabazitaxel. It often works when Docetaxel is no longer effective.

His pain is better and being better managed now. He is 67 y/o we live in Staten Island NY and he is going to Sloan Kettering in Middletown NJ

Good about the pain easing a bit. I'm a customer (no longer call myself a patient) of Sloan Kettering in NYC. Well he is at one of the best cancer hospitals in the US if not the world. I've seen customers who have travelled from all parts of the world. God Bless your Dad (and his whole family and friends)...

p.s. I've got dandruff older than your Dad.

I had a taxane based chemo and my PSA went up. I switched to a platinum based chemo and now it is going down! Maybe it's worth a try.

I have a BRCA mutation and had read that platinum seems to work better in this situation.

As Fran2020 said, cancer cells express their PSA at death. I was measured and rescanned after my 3rd cycle (of 6 total) so give it time to work. I am now on Jevtana (Cabazitaxel, the next gen taxane chemo after Docetaxel) and am being scanned after my 3rd cycle again.
