I have mCRPC with lymph node and bone mets. I am presently undergoing Lu177/Ac225 radioligand therapy in Heidelberg. I have been on Eligard for the past 15 months. Failed xtandi, zytiga, and chemo. My primary symptoms affecting my QOL at this time are muscle aches involving my R deltoid, L scapula, and lateral R rib cage. These pains were precipitated by only minor injury, and seem to be taking forever to heal. It has affected my time in the gym, as I don’t want to aggravate the injury. I don’t believe it’s bone met pain as I don’t require analgesics other than occasional ibuprofen, and I sleep all night without pain. Any suggestions?

Hi Rex, can you afford a physical therapist? If you have a medical plan it typically pays partly for it. When I had muscle injuries before I had cancer, my PT would actually have me stretch and lift weights with the affected muscle then use ice and electrical stimulation afterward and to do this 3 times per week. It seemed counterintuitive to me at the time but the PT stressed that I not rest the muscle.
Bone metastases can cause secondary pains in muscles and other places due to the release of cytokines. There is often a pain flare when a new medication is tried and the killed cells release cytokines. If an NSAID gets you through the night, that is great. Low testosterone and radiation slow healing.
I wish that you were in Heidelberg for pleasure .....only... a pretty place ... I think by sleeping all night pain free that you are promoting healing . After only RT and five years of continuous adt I’ve got pains and aches in joints from a to z..I think it goes with the show .. We are getting muscles and joints stripped . Good luck finishing and may it help you out . In all seriousness I hope that they heal you and improve QOL... ... I’d like to hear how you’re being treated ,is there snow on the ground?
Not presently in Heidelberg. I return for treatment #2 on March 17.

Sorry about your pain.... I have pain where I never knew that part of my body ever existed. I just live with it and laugh as much as I can. -

Yep! Inexplicable pain. Only my wife sees and hears my ouch ouch OOch 24-7...a way of life now... I take it as my lot after 50 yrs running the planet like I was king. I’m put in my place . Humbled ..and I Thank God ! For each sunrise 🌅

I haven't had Lu-177, but if it's anything like my experience with chemo, the slightest injury will hurt worse and take longer to heal. I continued "running" throughout chemo, but by the end it was barely a brisk walk. About two months after chemo was completed my body began recovering and my fitness returned with surprising speed. By six months I was running better than I had in several years.
My recommendation would be to continue exercising as much as you reasonably can during treatment, but it will be frustrating as it will become harder and harder to do less and less. There's still benefit in doing this, and it will help your body recover after treatment.
My two cents, your mileage may vary, etc., etc.
