Lost my < sign. PSA now 0.1. Is there reason for concern that the Xtandi has started to fail after 21/2 years.

I would wait for radiographic evidence.

Repeat PSA every 2 weeks for 3 months and chart and if you see a clear upward trend then act...I will not jump to any conclusion just by one PSA reading.

You really need to get some more results in, PSA assays are not that accurate, give it a few weeks and test again, then if it is still positive you need to get more tests so you have a PSADT, this will show you how fast growing / aggressive it is.

Yes. To soon to call. It could be just a glitch in the test. . I had that one time myself. You will see next time if there is a reason to worry about the monster

Agree with TA...too early to tell. After 4 1/2 years of blissful complacency on ADT (Lupron and-until recently-Bicalutamide) this year my PSA went from <0.10 to 0.15, to 0.22, now 0.45. Naturally, I was really worried. My Mayo M.O., on the other hand, actually congratulated me on staying so stable, and reminded me that the first number was still zero. He said that as long as I remained asymptomatic and there were no huge leaps in PSA, there is no reason to panic. He said we would likely add either Zytiga or Xtandi when the right time comes for it. I’m sure your oncologist will keep on top of it, too. Best wishes!

My first PSA from radiation treatment was <0.1. My 2nd, 3 months later was 0.1 - all my MD's were unnamous in saying the change can safely be ignored. You didn't mention if you had treatment besides Xtandi - if you had as a primary therapy radiation treatment, it's not considered unusual to see some small number PSA readings since there is still prostate material in the body. Some papers/reports show that a steady PSA state may not be reached for 1-2 years after treatment.

I've been 0.1 on Xtandi for a long time. Dr. said yesterday that it was actually less than 0.1. My alk. phos. took a 30 point leap once, but came down again over time. I've been on Xtandi for 25 months. You are probably just like me. Waiting for that shoe to drop. Relax. Maybe it will be a Jordan Why Not Zer0.2

Some good advice above. Too soon to tell. You have the luxury of time and sleeping peacefully. A raw food/smoothie diet may extend the end of this good run (or, with extra good luck, set you free). When/if an uptrend is confirmed, try all sorts of things before you get talked into using sledgehammers - you have lots of time to experiment with more gentle solutions first.
