recap..son age 40dx jan 2018..PSA 1250 metastatic bones and lymph .. lupron,xgevia and 12 rounds of chemo and zytiga. PSa dropped to 3 but now uo to 8. 4 months on zytiga. SOC treatment options are xtandi, more chemo orR223. Doc recommends trial at U of Chicago , phase 1/1b so all patients get Rucaparib (PARP inhibitor) and Nivolumab . please advise as we meet on tues to sign up .. any other advise or trial info would be helpful. thanks for this site as i read it daily and feel i have been very educated by all of you

I read that trial last night, but I thought a PARP inhibitor worked best (only?) for those with a specific mutation?

I am so sorry your son is only 40 and dealing with so much. I assume you've thought about radioligand treatment in another country? Does he not qualify for the VISION trial?

Phase one trials are pretty risky unless the trial has a history of spectacular results with low to moderate side-effects.. At this point, the R-223 or the "Vision" trial might be a better choice.. Also, the PSA doubling time is more important than the actual number..

Have you had any immunohistochemical or genomic tests done?

we have foundation one testing which showed nothing but it was done in 2018 . no immunotherapy

he had a biospsy done at northwestern for a trial last year and they did not get a viable same this trial requires a biopsy but whatever they find will not exclude or include you in tbe trial

If the tumor doesn't have a lot of PD-L1 receptors, it's not likely that Nivolumab will work. PARP inhibitors work best if there are mutations that interfere with DNA-repair.

To Eabradley... I'm sorry I cannot contribute any information regarding yours son's plight. The only thing I can do is to add him to my prayers. I wish him wind in his sails....God Bless him and you too.

thanks for all tbe responses. trial at U of chicago would be a real hail mary shot. looking to try the vision trial first ..hope he gets selected. what is the success rate of this and how much extra time does it buy us
