I'm new here. Happy to have found this group. Dx stage 4 at age 63 - 9 years ago. RP, Lupron, Zytiga, Doxetel, Xtandi, Radium 223. Doctor says I qualify for this trial. Anyone have experience with it? All treatments have failed me, so this is a last ditch effort to stay in the fight.

It looks like it may have some activity if you have the mutation "AR F876L." And it's possible that other markers of genetic resistance may be reversed by it too. It's a short (18 month) trial, and it looks like you've already had every approved medication except Provenge and Jevtana - so I don't see much downside.
Thanks for your reply. I did actually get Provenge early in year 3, I believe. No Jetvana. Not sure if I can take any more rounds at my age. I see many knowledgeable men here, much more so than I. Definitely worth a try. Trying to navigate this site and so far haven't seen any other info on this trial.
Good luck, Flintknapper. I am in same boat as you, dx in 2013, been through surgery, ADT, radiation, all meds, Taxotere & Jevtana. PSA went from 312 in March to 40.1 in June, but now PSA is 75.6. I believe my next step will be a biopsy (bone or liquid?). Just yesterday I began to experience severe & constant pain for the first time (been lucky up til then!) Dilaudid & Methadone aren't helping any. I will bring this trial up to my oncologist this Tuesday, though I am out of the area of any locations listed.
Flint- I am 10.6 psa and lots of bone mets. Zytiga, prednisone n Lupron which just started six weeks ago. I just wanted to tell you that I am impressed n inspired to know that you have fought this creature for over nine years! Any advice from the veteran to a rookie?

Fitzbruce1-to be honest - I never thought I'd still be kicking around this long. I guess if I could start over, I'd lose all the sleepless nights of worry. There seems to be something coming around the bend every time it looks like the end of the road. If I was just starting out now like you, I'd think I'd be around a very long time.
I am new here....I will list where you can find clinical trials...it looks like you have not had any of the PARP inhibitors or immunomodulators....Have you had any genomics testing to see what kind of markers may be available to attack.? They are using lynzparza(olaparib) and pembrolizumab(keytruda) and nivolumab(opdivo) in trials...there are some new vaccine trials also.... The NCI Match trial is solid and huge and will determine how many cancers will be treated in the future. I like the potential TRC253 and TAS3681 offer which might allow Lupron, zytiga, apalutamide to work again, by blocking AR mutations.
Hang in there...cyclic peptoids are getting started on and KAT6 inhibitors may be the answer for all cancers by putting cancers to sleep. Here is the trials site:
My unlucky story is to go from PT3A with no lymph node involvement to Stage 4--having my bone lesion removed by stereotactic radiation in 2 weeks. On zytiga and prednisone and Lupron--trying to deliver a knockout blow but reality is that prostate cancer is one tough, evil customer.

I was researching some of the info you posted and while reading about TAS3681, it appears the clinical trial is a rare find in that my husband may qualify despite the fact he's been in bone marrow failure and receiving RBD transfusions since his last (4th) Xofigo injection in December! He's had PC in his bone marrow since he was first diagnosed in early 2017, and a couple treatments have worked well to put him in remission (Docetaxel, then Lynparza + Zytiga) -getting life back to 'normal'! - but now that he's in bone marrow failure, he's disqualified from almost every treatment and clinical trial I find! It sounds like this one does not specify platelets or hemoglobin: clinicaltrials.gov/ct2/show... btw, I did a search on this thread to see if anyone else has brought up TAS3681, and nothing came up - not even this post of yours!
