I've experienced a bouncing upward of my Alkaline Phosphatase level after starting Zytiga, 5 weeks ago. My ALP jumped up to 111, just outside the normal upper limit. Apparently this is fairly common and along with a >50% reduction in PSA appear to be positive prognostic indicators.
I suspect the mets are healing and the bone is remodeling. I didn't check my ALP until 5 weeks after starting Zytiga so it may be on the way down now. My PSA sure went down though, now it's 0.1, down from 26.5 at the last check 6 weeks ago.
My bounce concerned my Doctor so he put me on 750 mg. Zytiga dose instead of initial1000 mg. All blood work returned to normal in a few weeks that was six months ago and still normal today. PSA undetectable. The good thing about my Doctors instruction to cut down meds is two fold. One it worked, and two the cost reduced my copay .
I haven't tried Xtandi. I'm on Zytiga now, but when it stops working (hopefully a long time from now) I will probably try Xtandi. My last PSA check on Zytiga was undetectable and there are no side effects so I'm going continue with it.
my husband ALP was very high and the doc here in Edmonton, Alberta didn't say anything ... until suddenly he stopped the drug completely in December ...
what's the next step if ALP is very high actually ???
