Check in check out sheet for elementary students

My daughter has the same thing at her school. She doesn't get a treat from the teacher she check in with. Shes in 2nd grade her check teacher is her first grade teacher. She sees her in the morning to set goals and get motivated and see her at the end of the day to add the totals. I make sure she takes her meds an hour before school. And i make sure she eats breakfast at home. But she takes a afternoon pill also.

Hi, I like it! My 13 yr old son, in 8th grade, has some sort of "point sheet". He hands the sheet to each teacher through the day and they "score" it. I think he starts with 10 and pts are lowered based on behaviors. Then all pts are totaled at day's end. Once he reaches a certain total he can get some sort of 'prize' from his Special Ed teacher. I've been meaning to get a copy of it and suggest any changes that may serve him better. Thanks for sharing yours.
