Advice for my 4 year old please!

Hello moms, ok so my 4 yr old son was diagnosed with adhd, anxiety and ocd. My main concern is he has multiple melt downs a day. When they are really bad they last 45 minutes. The worst is when we're in the car and he unbuckles and gets out of seat. Im so stressed out and scared that i cant help my son. We start therapy tomorrow not sure what to expect he doesn't like strangers. Does anyone have any advice for help with his meltdowns? If we're home i try and grab him up, put him on my lap with thomas (favorite blanket Thomas the train, he calls it Thomas) and hug him tightly sometimes it works but not all the time.

An idea is to work with the therapist and figure out what is causing him to have the meltdown - his anxiety or his behavioral impulsivity from ADHD (or both). This will help with the therapy.
