Due to low blood pressure and a bleed after a biopsy my fistula has clotted and is very painful it is also very swollen.Apart from take pain killers what else can i do to reduce the swelling and pain?

You don't say whether you are still using it, if you are then a fistulargram should remove the clot and allow you to carry on using it.
Is the swelling in the fistula vie nor in the limb? A clot shouldn't cause pain and swelling in a limb as my understanding is that a fistula is made ensuring that it is never the only blood supply to the limb.
I'm presuming you've contacted your doctor, if not in my opinion you should do so as soon as you can.
With regard to the pain - I've found best oral pain relief is paracetamol and nephapam taken together every four hours and try to watch a good film to take your mind off the pain.
get your renal doc to look at it. I recently had my old fistual ligated ( tied off) and got thrombo-phlebitis. This meant a course of antibiotics!
