Hi everyone can anyone please help on test results

My thyroid test results are serum free T4 above range 26.0 and TSH level below range at 0.06 my doctors seems to think this is fine and a retest in 6 months. So why am I freezing cold all the time, loosing weight and anxious making me miserable. I am on 100mg levythyroxin for many years.

First question: do you have Hashi's? If so, that could explain your high FT4, because you're only on a low dose of levo which is highly unlikely to produce an FT4 that high.

The other explanation is that with age, your conversion of T4 to T3 has got worse. T3 is the active hormone that causes symptoms when it is low. And I'm guessing that yours is low because in your last post you talked about high cholesterol. High cholesterol is usually caused by low T3.

All that is, of course, assuming that you left a gap of 24 hours between your last dose of levo and the blood draw? Otherwise, your FT4 is high because you just measured the dose you just took. :)

Thank you for reply, yes my last dose was just over 24 hours prior to test, and my cholesterol is slowing coming down. They don't do antibody test so hashi not been even considered.

Very important to know if you have Hashi's because of anomalies like this. But doctors don't understand the importance. If I were you, I'd get my antibodies tested privately because it looks like a possibility.

Do you always get same brand Levothyroxine at each prescription

what vitamin supplements are you taking

When were vitamin D, folate, ferritin and B12 last tested

Just testing TSH and Ft4 is completely inadequate but all NHS typically does

Suggest you get FULL thyroid and vitamin test done

Last dose at least 24 hours prior to test. B12 never tested to my knowledge, spoke to doctor he said being cold all the time is not thyroid related full stop. I can't win really

Exactly what vitamin supplements are you taking

Low B12 extremely common when over 65 and especially when on levothyroxine

Is your hypothyroidism autoimmune

you should have had thyroid antibodies tested at diagnosis of hypothyroidism

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high TPO and/or high TG thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Significant minority of Hashimoto’s patients only have high TG antibodies (thyroglobulin)

NHS only tests TG antibodies if TPO are high

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

ALWAYS test early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 3-5 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In days before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12 if last test result serum B12 was below 500 or active B12 (private test) under 70
