Querying diabetes

Hi

I have been suffering from episodes of light-headedness, I assume due to dehydration. For the last week or so, I have a permanent dehydrated almost headache. Except Thursday, when I was in so much pain I couldn't keep water down. Despite the feeling of thirst, dry lips, and headaches, I'm drinking a lot and peeing well (clear). I can't get the fluid to stay in my body, I've even tried using a rehydrate solution. I have also developed a sweet tooth, which I am fighting! Yes I'm tired and my skin is dry and I'm sleeping badly - but I have Hashi.

I asked for an emergency GP appointment (24th January) as my mother is a type 2 diabetic. In the meantime, I have ordered an HbA1c test, as it is time for my 6-monthly thyroid and vit levels which I do privately (and there is a test that does both).

I prefer to be prepared and do my research. Is there anything I need to know in advance, such as the effect of diabetes on thyroid levels? What can I expect?

Is this why I feel so crap all the time.

Sugar cravings suggest adrenal problems/low cortisol. Have you had that checked?

My cortisol was fine October 2023. It's not been tested since then.

But what do you call 'fine'? When did the sugar cravings start? Since October?

Yes, sugar cravings are recent, last few months. Been getting wrose as time goes on. Fortunately, most cakes have wheat in, and I wasn't not a big chocolate fan, but have found myself eating the stuff in recent months. Christmas is not the best time to develope that liking!

I have a relative with Diabetes Insipidus. I'm not saying you have this condition but it may be worth looking into.

It's not related to Diabetes Mellitus despite the name. The condition had a name change recently Arginine Vasopressin Deficiency or Resistance.

Some of your symptoms that you describe, dehydration, peeing a lot and being thirsty all the time can be associated with this condition.

How much levothyroxine are you now taking

Your post 11 months ago showed you were very likely on too low dose levothyroxine /T3

Free T3 4.0 pmol/L (Range: 3.1 - 6.8)

Free Thyroxine 12.5 pmol/L (Range: 12 - 22)

Thyroglobulin Antibodies X 329.00 kIU/L (Range: 0 - 115)

Thyroid Peroxidase Antibodies X 562.0 kIU/L (Range: 0 - 34)

Many members on Levo plus T3 find they need BOTH Ft4 and Ft3 at least 60% through range

Free T4 (fT4) 12.5 pmol/L (12 - 22) 

Ft4 only 5.0% through range

Free T3 (fT3) 4 pmol/L (3.1 - 6.8)

Ft3 only 24.3% through range

Then 4 months ago

I’m on Accord levothyroxine 75/100 mcgs, (although Endo would like to see this reduced to 75 daily because of low TSH result.)

TSH 0.18mU/L (0.34-5.6)

FT4 11.1pmol/L (7.7-15.1)

FT3 5.2pmol/L (4.3-6.8 )

Free T4 (fT4) 11.1 pmol/L (7.7 - 15.1) 

Ft4 improving at 45.9% through range

Free T3 (fT3) 5.2 pmol/L (4.3 - 6.8)

Ft3 only 36.0% through range

Did you increase in Levo to 100mcg daily

And have you got any more recent results

Are you on dairy free diet as well as gluten free

I'm presently on levo 100mg daily. (GP thinks its 75/100 alt days, but I have increased it. ) This is what makes me feel most well. But GP takes one look at the suppressed TSH, panics and decreases my levo prescription. Only once was my T4 over range , and only just 103% through range, (if I remember correctly). In my opinion, that isn't enough to worry about it causing a long term health risk, compared with my quality of life.

Liothyronine didn't suit me. and the consultant halved my levo when she put me on it, because my TSH was suppressed on 100mg daily.

I haven't tried dairy free. I know I should, but I look at cheese and ...

I have ordered an at home test, and will do it as soon as I can. I stopped my B vits ready for the test.

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12 if last test result serum B12 was below 500 or active B12 (private test) under 70:

So are you no longer on any T3?

You results showed you need increase, not reduced/stopped

TSH is almost always suppressed on T3, even on inadequate dose

I resisted dairy free for a long time

Gluten free since 2016

Dairy free from 2022

Like you I couldn’t imagine life without cheese. But it’s fine. I do use plant based “cheeese” by cathedral city on Shep pie or spag Bol or similar (very good)

Goat or sheep cheese is fine (different enzymes) so proper halloumi is “allowed “

There’s loads of dairy free/vegan ice cream, yogurt, mousses etc

And vegan chocolate

That's a scary thought, my TSH was suppressed before T3 was added. Unless my T4 is below 15% through range its lower than she's happy with.

Sounds like you need a new endocrinologist

Lots of thyroid patients have sluggish or poorly responsive TSH

Dose should be adjusted looking at Ft3, Ft4, vitamin levels and symptoms
