I've got to have my Parathyroid levels checked again following an abnormal result in August. I know with thyroid we test early morning , fasting and no thyroid meds.
I'm wondering if Parathyroid levels also fluctuate during the day and if so is there an optimal time to book a blood test or if it doesn't matter? Are they affected by food, medication etc?

Hi Sparklingsunshine, I imagine that they will also be checking your calcium levels to compare with PTH. How about trying to get a test at about the same time as you had your other done? Then you will hopefully be comparing like for like? Good luck.
I'm just looking at this for myself as I'm having issues with high PTH and have to get repeat blood tests done including calcium and vitamin D.
My calcium is normal(?) not sure of levels yet, but my vitamin D was low (working on it!) and PTH high. Also think I may need to request a DEXA scan (?) as broke a bone easily two years ago.
ENT said for me to get a scan of kidneys too as have left side ache.. All new territory for me so when I find research/info that may be relevant and helpful for you I'll post them.
It can maybe point to secundary hyperparathyroidism (due to a lack of vit D) Have your IONIZED calcium checked (with PTH, vit D), if that is too high and your PTH is high, there is a problem (might be primary hyperpara due to a benign tumor, adenoma in the parathyroid gland).
My daughters test results fluctuated wildly and the only way we got a diagnosis was anitrasound showed an adenoma on one of her parathyroid glands which needed surgery.
Hello, yes, I too am having some problems understanding the relationship between parathyroid-hormone, Vit.D and Calcium, and what’s significant. My parathyroid hormone was 8.69pmol/l and the normal range was given as 1.95-8.49. That doesn’t seem particularly high, and my calcium and Vitamin D tests showed as slap in the middle of the normal range. These readings were all taken from the same blood-test. I also have Crohn’s, osteoporosis, osteoarthritis and CPDD/psoriatic arthritis. I’ve been put on a very high dose of Vitamin D for 5 weeks prior to further blood tests, and an appointment with a metabolic bone rheumatologist.
A couple of parathyroid links which just might help:
Parathyroid.com
The most comprehensive information about parathyroids on the internet. Despite being from by a commercial organisation, it is produced as a source of information for patients with promotion very much a by-product.
Parathyroid.com is the world's top source for parathyroid glands and their most common disease: hyperparathyroidism. The site is maintained by the Norman Parathyroid Center, the world's leading parathyroid surgery center performing about 10% of the parathyroid operations in the United States--nearly 4000 per year. Hyperparathyroidism, parathyroid surgery, and blood calcium levels are discussed in great detail.
Dr. Norman of parathyroid.com is a very commercial organisation indeed, he has a private clinic where only parathyroidectomies are performed. He also got a hefty fine of $4 million to resolve allegations that he violated the False Claims Act by knowingly engaging in various unlawful billing practices. It makes you question the integrity of it all... and it makes you prudent.
