Fatigue on T3 initiation

hi all, started Lio 5 mcg without change to my levo 100 mcg. Reason i added that T3 is on lower side and T4 on higher side, Reverse T3 higher than range. All vitamins and minerals good. tests taken at right times as shared frequently on this forum. Suffering from low mood and anxiety.

Did anyone else experience fatigue/sedation on initiation and did it settle down?

Lots of us initially found adding T3 would almost immediately make you feel sleepy

That effect does seem to wear off after few weeks

Small dose T3 at bedtime often improves sleep

You could try taking as 2.5mcg twice a day instead of single 5mcg dose

The adrenals aren't used to the extra help so that is why you may experience either restlessness, palpitations, anxiety on one hand or feeling more sleepy or tired on the other. It usually takes a few weeks for the adrenals to adjust.

I’ve had a theory (admins will say if it’s too far out in left field).

You started, out with high T4 and high reverse T3, so you are converting T4, just to the inactive analog of T3. So you add some liothyronine. Might this cause even more of your T4 to go to rT3 instead of Free T3 so that in the end, despite adding liothyronine, you may actually have less Free T3 on board?

On the cancer boards in the US, some people have reported better FT3 levels when they lowered their levothyroxine dose. We do tend to suggest lower T4 levels for cancer patients after TT (in the lower 25-50% of range) then are frequently advised here while having FT3 in the upper 25% of the range. It’s hard to know without, knowing your specific levels and ranges, but you might consider a small levo decrease.
