Extremely confusing bloods from June and last week. Help.

Hi all, i hope your all well..

I need help understanding my results as my latest are super confusing. I thought I was hypo but now I'm showing as hyper (I think)

June results show my antibodies but my gp didn't ask for antibodies this time, no idea why.

Can someone tell me what's going on with me, please. I've attached my results.

Presumably you are not on any replacement thyroid hormones?

you need folate, B12 and ferritin levels tested

Looking at previous posts symptoms started after head injury

You need to see pituitary/thyroid/adrenal specialist

BEFORE any consultation get FULL thyroid and vitamin testing

TSH is not responding well to low Ft4

Could be low vitamin levels

Could be some degree of central hypothyroidism as well as Hashimoto’s

For full Thyroid evaluation you need TSH, FT4 and FT3 tested

Also both TPO and TG thyroid antibodies tested at least once to see if your hypothyroidism is autoimmune

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high TPO and/or high TG thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Significant minority of Hashimoto’s patients only have high TG antibodies (thyroglobulin)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Essential to test vitamin D, folate, ferritin and B12

Lower vitamin levels more common as we get older

For good conversion of Ft4 (levothyroxine) to Ft3 (active hormone) we must maintain GOOD vitamin levels

What vitamin supplements are you taking

Also VERY important to test TSH, Ft4 and Ft3 together

What is reason for your hypothyroidism

Autoimmune?

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

Hi, I'm south Manchester and already under an endocrinologist at Salford (since March). He hasn't been too helpful as I had to insist my antibodies to be checked on my second appointment in june. He was happy to not test anything even though I was symptomatic of hypo and begging for help. My first appointment was in March where all the tests were done, apart from T3 and antibodies. I can attach if it will help build a better picture?I'm having my thyroid scanned this evening, so I hope it will help with a diagnosis because I'm perplexed as to what's going on as I thought I was autoimmune (hashimotos) and now I'm not sure. Is it pituitary damage from my head injury after all? My endocrinologist has stated that it's definitely not but I've zero confidence in him.

On page 2 my blood count is indicating hypo (MCV&MCH)

I just wanted to understand as I have a telephone appointment with my gp on Wed morning to discuss and I want to be able to advocate for myself as best as possible, my experience so far has been pretty awful.

Its been 6 long years and I really would like some relief/life back.

These results don't relate to being hypo, more likely low B12 or folate and I cant see any results for these.

What sort of diet do you follow? Do you eat red meat?

TPO antibodies are slightly raised indicated autoimmune hypothyroidism or Hashimoto's. This can start off with quite widely differing results as the thyroid is slowly destroyed. It can take a number of years before Levo is required.

Your FT4 is on the low side yet TSH not that high. Suspect you have other low vitamin levels other than vit D.

Get GP to test B12, folate & ferritin, start a new post when you have the results.

High MCV - most likely low folate/B12

Your high mcv could indicate b12 or folate deficiency. Try not to take b12 or folate supplements before testing and get the test ASAP. This is because supplements will skew the results and your GP will then be v reluctant to diagnose because this would require the surgery to do some work ie injections.

Hi,

I stopped all vitamins over a week before my morning test as I'd read on here biotin can affect the results, so my results are with no vitamin supplements in my system at all.

What do you think my gp will say? It's not an urgent consultation (the receptionist said so)

Getting tested for possible b12 deficiency is pretty urgent. Alternatively you can do it privately - there are lots of labs to choose from. Re b12 supplements including a multi B these will skew any result for weeks - but I don't know how many. The PA website likely has this information.
