I have been on 112 Tirosint for 3 months. I am apathetic, irritable, sleeping a lot and have crazy muscle pains. Based on below labs, I convinced my doc to add in T3. She dropped Tirosint to 100 and said add 5mcg of liothyronine. Should I give it a couple weeks on just 100 before adding in t3? Should I split t3 in two doses? I take Tirosint at night. Labs were done a week apart in June.

6/13:

TSH (0.465-4.68): 0.903

T3 (2.8-5.3): 2.5

T4 (0.78-2.19): 1.43

6/19:

TSH (0.465-4.68): 1.35

T3 (2.8-5.3): 3.1

T4 (0.78-2.19): 1.39

Reverse T3 (9-27): 15

I just feel so blah :(

A few days on 100 mcg levo rather than a couple of weeks.

I don't really think she should have reduced the levo at all because your FT4 isn't very high:

FT4: 1.39 pmol/l (Range 0.78 - 2.18) 43.57%

And taking T3 is going to lower your FT4, anyway. So, your FT4 is probably going to end up being too low.

Personally, I don't think there's much point in splitting 5 mcg T3. It's such a tiny dose. Hopefully, she'll be increasing it in a little while because it's not going to bring that very low FT3 up very much.

I do not take T3 at all. But I have to question whether you should have reduced the Tirosint dose at all! You are far from top of the Free T4 range.

Also, you cannot expect a useful and meaningful change of anything related to thyroid in just six days! Not clear why you had that second test on the 19th?

Ah - right. RT3 testing isn't much favoured in the UK. In fact, almost unobtainable and the few who offer it send samples to the USA - and it costs a fortune!

what vitamin supplements are you taking

What were B12, folate, ferritin and vitamin D results at last test

Test at least annually

Many on replacement thyroid hormone find they need to supplement vitamin D, separate magnesium and vitamin B complex daily and continuously

Some people also need separate B12, especially initially

Some also struggle to maintain good iron and ferritin

If B12 is low, you may need a separate B12 supplement as well as a vitamin B complex

Does that mean isolate b 12 from the B complex?

How did you all figure out if you need more T4 or more T3?

I see people writing here, “if my FT3 was that low/high, FT4 that low/high, I’d feel very ill”......
