Could be Hashimotos?

I'm from the UK but based in the US currently. I have family history of both hypo and hyper thyroid with all the women on my mum's side of the family. Last year I covid and then two months later my left ovary and fallopian tube removed due a large cyst. I then had my TSH and T4 tested a couple of months after the surgery and the results were 0.65 (range of 0.55-4.78) and 0.99 (range of 0.89-1.75).Over the last year I've been experiencing the following: very dry eyes that are sensitive to light, hives usually after scratching an itch, I had an eye twitch that last several weeks, occasional hand/finger tremors, red blotchy patches from sun or heat, brain fog, fatigue and feeling like I haven't slept even thoigh ive had 7+ hours, bruising easily and bruises taking a long time to heal, darkening of elbows and navel, very heavy periods, constipation unless I live on vegetables, migraines, teeth grinding in sleep, thinning of hair (although that has slowed down), chest tightness, post-exertional malaise and muscle/joint aches, dry hair, dry skin on legs despite moisturizing every day. I was also diagnosed with POTS last year.

My GP did thyroid blood work last week and my results were as follows:

TPO

67 IU/mL range = 0-34

Thyoglobulin Antibody

<1.0 range = 0.0-0.9

TSH

0.84 uIU/mL range = 0.34-4.94

T3 free

2.7 pg/mL range = 1.7-3.7

T4 free

1.03 ng/dL range = 0.70-1.48

I know my TPO is only slightly elevated but my TSH also seems fairly low for the normal range. My Dr says there is no need to do anything more at this stage other than monitor it once a year given family history of women all with hypo. Should I push back? Anyone else with Hashimotos have these kind of results?

Have you ever had your cortisol levels checked?

I don't believe so, no. Is that related to thyroid issues?

Not exactly however some of your symptoms could be a sign that your adrenals are struggling. I’m afraid I don’t have a great deal of time to respond right now but might I suggest that you read up on Addison’s disease? Also worth having a rummage on this forum for posts discussing saliva cortisol tests. I’ll check in again this evening, work beckons!

Your mention of dry eyes and skin made me think of Sjogren’s syndrome, especially as you also have POTS (Sjogren’s and autonomic dysfunction often co-occur). Just something to keep in mind if you don’t find another explanation.

Have you got a treatment plan for your POTS, and is it helping? I know POTS can’t explain all your symptoms, but if you can improve the POTS ones it may make the situation more clear.
