Do you normally split your dose

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

NDT ….day before test split as 2 or 3 smaller doses spread through the day, with last dose approximately 8-12 hours before test

No vitamin D or ferritin results?

I did all that before testing as you advised. Based on my results do you think I need to up my dose?

Vitamin levels are good

FT3 false low as last dose was more than 12 hours before test

You might experiment with small 1/4 grain increase?

Or consider adding 25mcg levothyroxine

Hello BigFraz :

How long have you been on 2 +1/2 grains and is this a split dose or just 1 dose and how long before the blood test did you take your last dose ?

I normally just take in the morning as I haven’t noticed much difference if any split dosing and I have so many other dings to take. Last dose was 20 hours before bloods. Vitamins stopped week before.

OK - so we usually recommend taking a fasting blood test around 12 hours from last dose and track on the T3 reading - so this is likely a non fasting blood test at 20 hours and now showing your T3 at a ' false low ' :

How do you feel ?

Looking back I think I saw a similar post from around a year ago on this same dose and see you are still dealing with gastric issues which might be causing absorption issues - a common issue when dealing with Hashimoto's AI thyroid disease.

Many forum members follow the research and suggestions of Dr Izabella Wentz regarding Hashimoto's and food sensitivities who writes under the website :-

Ok. I’ll remember that in future.

All symptoms are gone, I’d say apart from the gastric issues. I do have sibo and gut dysbiosis which I’m trying to get rid of . Maybe if I deal with this my gut issues will remedy. Thanks

Well yes as the thyroid hormones are broken down and absorbed in the gut - so as your gut function improves you may well find you do not need as much NDT :

However with this AI disease the immune progressively destroys the thyroid and ultimately with the patient needing more thyroid hormone support rather than less - but maybe your thyroid now fully disabled, and the incidence of these AI attacks gone along with your thyroid and this about the right dose for you longer term.

as you have Hashimoto’s are you on absolutely strictly gluten free diet and/or dairy free diet

Or tried them

I’ve tried all that it doesn’t seem to make a difference. I’ve actually been on an almost carnivore diet for 5 weeks with a few vegetables because I’m trying to starve the overgrowths whilst I take antimicrobials, but still feel pretty much the same. If my medication is the correct dose then I think I’ll need to just concentrate on getting rid of the sibo and take it from there. Thanks

I would agree

Getting rid of SIBO is first step
