High to Low TSH?

Hi everyone,I have recieved my Thyroid bloods back last week and I am so confused 😕 as you can see in attachment, the latest results.

TSH - 0.11

T4 - 21.8

(Never do a t3)

In January I was

TSH - 29.51(TSH been high a long time)

T4 - 11.3 (in range)

also in December I got a positive result for antibodies.

I am 20+ years in with this condition and this is the most confused I have been. My GP noted the results as normal nonfurther action? I stopped my levothyroxine just over a week ago as I was having palpitations the day I had the test done. I was great for the first several days but the last few days I've felt not good again. I see my endocrinologist soo but don't know what to do about my levothyroxine as my GP doesn't have a clue.

Any advice would be very much appreciated. Thank you in advance.

Seems to me that you were over medicated so perhaps dropping back a bit might have been more advisable than just stopping completely. Your GP does indeed seem a bit clueless looking at your first post. I’d starting taking your levo again but not the 200mcg that you were on, perhaps 150mcg until you’ve seen your Endo.

I have an emergency apt on Thursday asbits the same specialist who treats me on my insulin pump. Thank you for your reply. I have taken a small dose of levo now.

Stopping your meds completely not a good idea.When T4 is dropped for some reason the T3 levels increase for a short time which is why you felt good for a few days.You haven't had your T3 tested for a while? Maybe you are not converting well but impossible to tell without lab results.200mcg is quite a big levo dose.

I eas just a bit scared how I was feeling as I can't get a GP apt for weeks, a phonecall takes up to two weeks and the reception staff at the doctors go by the reports so mine said 'Normal no forthright action'

The receptionist told me if the GP was concerned they would have stated it 😞

If your antibodies are high, you have Hashi's - aka autoimmune thyroiditis. When you have Hashi's there's no point in looking for logic in your blood test results because they can jump around. Looks like you're have a temporary Hashi's 'hyper' swing, which is nothing to do with your dose. The best thing you can do is stop your levo for a while until you feel hypo again.

When you're back on your levo and been on the same dose for six weeks or more, the best thing you could do would be to have a private test to see how well you convert. Hashi's people are often poor converters. But, you need the FT4 and the FT3 tested at the same time to find out. :)

I have felt different ways different times since around November and being type 1 on an insulin pump it's effected my bloods a little to. I just want it resolving. Thank you for ypur reply.

That link was really helpful greygoose thank you, I can also get a private scan and the blood tests are really fast. That link will be valuable to a lot of uk worried people I think.

How old are you - thyroid often goes haywire around menopause

Do you always get same brand of levothyroxine at each prescription

Levothyroxine is an extremely fussy hormone and should always be taken on an empty stomach and then nothing apart from water for at least an hour after

Many people take Levothyroxine soon after waking, but it may be more convenient and perhaps more effective taken at bedtime

No other medication or supplements at same as Levothyroxine, leave at least 2 hour gap.

Some like iron, calcium, magnesium, HRT, omeprazole or vitamin D should be four hours away

(Time gap doesn't apply to Vitamin D mouth spray)

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

When were vitamin D, folate, B12 and ferritin levels last tested?

What vitamin supplements are you taking

Have you had coeliac blood test yet?

Get tested BEFORE trailing strictly gluten free diet

I have been treated for hypothyroidism for 20+ yrs and everything was fine (even after my early menapuase) up until I got poorly in 2018 losing 28kg with a double duct dilation suspecting pancreatic cancer, I was fortunately diagnosed with Pancreatic Endocrine Insufficiency and started on Creon, then followed by Tupe 1 diabetes mielitis, esophageal dysmotility then Autoimmune inflammatory arthritis. My TSH was constantly really high barely fluctuating my levothyroxine increased slowly yo my now dose.

I was started on Methotrexate injections for my AIA (PsA) and my Thyroid results started becoming normalised (ish) then December I had positive antibodies result TPOa and from there its gone haywall.

I do take vit D and other meds but I take levo every morning 7.30am with mu pain relief for my AIA but vit D and other meds are taken later

I haven't changed anything I do since initially starting on levo just my disease has changed. And a lot more since 2022.

You must ALWAYS take levothyroxine on its own on empty stomach and then nothing else apart from water for at least an hour

What was last result

Aiming for vitamin D at least over 80nmol

Vitamin D test twice year

Test vitamin B12, folate and ferritin at least annually

Hi, I take Colecalciferol 800unit tablet a day.My last Vit D results attached. I've checked back to 2022 and no B12 or foliate tests?

The hospital told me it was okay to take my pain relief on waking and levo but not vital d or other meds as I also have insulin o a morning feom my pump.

I can't remeber the last time I had my B12 tested but my last vitamin D was normal but I take prescribed vit d.
