Endo no longer wants to see me.

My recent blood tests show my tsh is 0.54 (0.35 - 4.94 ulU/ml) and my t4 is 14.5 (9.0 - 19.0 pmol/ml). I am on 75 mcg euthyrox. I had a scan on Friday and now have residual thyroid tissue on both the left and right side, 6 months ago I only had thyroid tissue on the left side. Total thyroidectomy was done in July 2022. My endo said I no longer need to go to him anymore and any scripts I can just get from my local doctor. He wouldn't discuss the long term prognosis of the thyroid tissue regrowth.

just testing TSH and Ft4 is completely inadequate

FT4: 14.5 pmol/l (Range 9 - 19)

Ft4 only 55.00% through range

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Is this how you do your tests

How do you feel

For full Thyroid evaluation you need TSH, FT4 and FT3 tested

Also both TPO and TG thyroid antibodies tested at least once

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

Hi slowDragon,Thank you for replying, I did the test at 8.30am and meds previous morning. I take selenium, magnesium, zinc, vit B12, vit A, boron, vit c and d. I feel tired, get heart palpitations and feel very hot especially at night.

P-glucose 4.9 (3.3 - 6.0 mmol/L)

Vitamin D25 18 ng/ml

S-calcium 2.41 (2.10 - 2.55 mmol/L)

Anti tpo 137.18 (<5.61lU/ml)

Anti tg 21.3 lU/ml (<4.11 lU/ml)

What was reason for thyroidectomy

Your high thyroid antibodies confirms autoimmune thyroid disease

Request 25mcg dose increase in Levo now

Or get full thyroid test now including Ft3 and vitamins first

You also need to get coeliac blood test done BEFORE trialing strictly gluten free diet

Presumably you aren’t in U.K.

Vitamin D is insufficient

How much vitamin D are you taking

Aiming for 40ng/ml or 100nmol

I was allergic to the meds for hyperthyroidism and I had an enlarged thyroid gland with t4 at 61, the endo sent me to a surgeon to have a tt as i was told I would die if I didnt have the procedure. I had test done in October with my gp for heart palpitations at night, sometimes I cant lie down so I sit and wait for the feeling to pass, I usually take cayenne pepper to ease it. I will ask my gp if he will help with an increase in my meds. I live in South Africa.

S-vitamin B12 350 (>138 pmol/L)

Anti nuclear factor- negative

DsDNA antibodies (crithidia) - negative

S-IGA (g/L) - 1.65 (0.41 - 3.49 g/L)

TTG IgA - 0.3 ELI u/ml (7.0 ELI u/ml)

Anti Gliadin IgA Ab -1.0 u/ml (<7.0 u/ml)

DHEA-S - 1.1 UMOL/L (0.8 - 4.9 umol/L)
