Quick answer as you’re seeing doc. Really emphasise ongoing symptoms and press the for endo appt as I feel you need some liothyronine. You may have to go private..
I have had under active thyroid for over 30 years asked repeatedly to see an endo, got a new doctor that did this for me was over the moon.
But went to doctors for a different reason she said I was in the wars, I said I had an endo app referral going through she said think we have had a reply to that they do not want to see you, just keep TSH 2 or under. I WAS GUTTED no body even told me.
Can I request it again have I a right to actually see one, or is that the , just start on the private route.
OK - your inflammation is low but your ferritin looks very high - so looking back this seems to have been discussed already on past posts - can't see a B12 or vitamin D ?
Your TSH is well in the range :
Your T4 is at around 91% through the range but your T3 at only around 22% through it's range and showing you struggling to convert the T4 into T3.
The accepted conversion ratio - once T4 is up in the top quadrant - is said to be :-
1 / 3.50-4.50 T3/T4 - with most of us feeling at our best when we come in this ratio at 4 or under.
So if I divide your T4 reading 21.10 by your T3 reading 3.90 - I'm getting your conversion coming in at 5.40 showing very poor conversion.
Since your T4 is almost at the top of the range - the logical solution is to add a little synthetic T3 - Liothyronine to your prescription with a lowered T4 dose to restore T3/T4 hormonal balance.
Your doctor only has anti depressants as his back up suggestion to T4 monotherapy and you will need a referral to an endocrinologist to assess your need for T3 as unfortunately we do appear to be in a post code lottery where ICB/CCG financial constraints are deemed more important than patient health.
Thyroid UK - the charity who support this forum - hold a list of patient to patient recommended thyroid specialists and endos both NHS and Private - contact admin at thyroiduk.org for the list so you can hopefully request a referral to a recommended doctor - and distance doesn't necessarily need to be a problem as many specialist still offer video consults.
Hope that helps a bit - I haven't fully read your thyroid journey as sense you were getting anxious so may have missed something on a previous post.
Means very little without the necessary T3 and T4 readings though fully understand that's all you get in primary care -
I gave up in the end and buy my own full spectrum thyroid hormone replacement as the stress of trying to be heard within a system of brick walls at every turn - simply exacerbated my symptoms - and after over 2 years of delays and denials and presume discrimination on age/sex - gave up and have been looking after myself for the past 5/6 years.
I do not have a thyroid having Graves Disease had RAI thyroid ablation back in 2004 :
Contacted roseway labs and have managed to get SR T3 10 mcg
With a consultation and subscribe.
£55 consultation fee
Prescription £80 for 100 tablets SR T3 10 mcg.also have set up repeat for another 3 months , but have to send her bloods at the end of the first 3 months to see how I am fairing with it.
She said they make the compound up in there labs.
She was lovely, so any advice would be grateful , she has warned me I could have some side affect palpitations etc.
Hair is chronic now doctors are not bothered at all , would low T3 cause hairloss and chronic thinning, I have bought a wig now but still do not want to leave the house.
I don’t know about hair thinning but low FT3 caused severe depression and whole body pain in me. I eventually (after years!) saw a psychiatrist who prescribed liothyronine. But if you persevere you should find a friendly endo. 🤞🏻
If you don't reply using the button directly where someone has responded to you then that person doesn't get a notification, for future reference.
I can't see where you have given the answer to my questions. TSH is highest at 9am or earlier so we recommend booking thyroid test for then.
Testing at different times of day will give different TSH results.
I'm sorry that your referral has been refused. Often unless you are referred to a specific Endo (which is difficult if not impossible) they are not pro T3 anyway which results in a long wait for nothing. Indeed exhausting.
I suggest you email info@thyroiduk.org for a list of T3 friendly Endo's. Choose a couple then make a new post asking for feedback on them by personal message.
