please I don’t understand please explain why my T3 dropped

on thyroxine 100mcg not T3

T4 23

T3 3.5

May 2022

T4 19

T3 2.2

medication the same one is higher t4 but the difference of 19 to 23 is not a lot but a huge gap from 2.2 to 3.5

is it as simple as I had more thyroxine so it raised the T3 ?

Now present day

I am now on 30mcg T3 125mcg t4

My last test

Also my b12 is double.

my vitamin b co strong so not contain b12 I read it could be blood cancer I’m freaking out

Each tablet contains the active substances: 20mg nicotinamide, 2mg pyridoxine hydrochloride, 2mg riboflavine and 4.85mg thiamine mononitrate.

Test was delayed -

27 hours after thyroxine and 13 hours after T3

Note

I have small bowel paralysis in parts or digestion issues so vomit undigested food in evening so not sure if evening doses is not absorbed

Second - cortisol is raised ? My morning last test was 697 and max was 525

Severe weight gain on legs

Also I read that some people too muxh t3 made constipation?! I’m totally constipated all the time

Very stressed

As you said, the test was delayed so it's difficult to make a direct comparison to the previous test. Your T3 level will have dropped a lot after 13 hours.

For the previous test what was the timing that you last took T3?

This graph illustrates what happens to your blood level of FT3 after you take a tablet. If you have a blood test done after a different amount of time you are measuring a different thing.

I am still confused why b12 so high and why T3 dropped could a higher thyroxine make my T3 drop?

If it helps you, my B12 was over 2000 and it really doesn;t matter as it is a water soluble vitamin and anything our body doesn't need it pees it out. Relax about the B12, honestly.

I don't see a result for ferritin? Your other vitamins look OK.

Probably you need some more T3. Constipation is a very common hypo symptom.

Retesting at the correct timing of taking Levo 24hrs and T3 8-12 hours is a great idea and will hopefully put your mind at rest.

I was left for 5 years alone In bed and wasn’t until I checked my T3 it came out at 1.8 !! Another year to wait for medication

My ferritin was only 35 last test.

Taking a B Complex that contains B12 will skew your B12 result.

you seem to have two questions .

1)T3 drop .

2) raised B12 / ALT /Serum Alkaline Phosphatase.

the title of your post "why has my T3 dropped" is meaning people are discussing old T3 results that are no longer relevant.

The T3 difference on those old tests ,is probably just because when the T4 was higher therefore the T3 was also higher .But you are now taking Levo + T3, so those old results are no longer relevant .

We can't know if your T3 NOW is lower, or higher than it was previously, because there is no range given for the old T3 results , and we don;t know if they were total T3 or Free T3.

is the image a current test ?

if so, it shows a very raised B12..,,(and i think you are saying you do not take any B12 at all ? your wording is not very clear so can you clarify please)

It also shows a raised ALT.

and a raised Serum Alkaline Phosphatase.

I don't know if these results are concerning, or not.

I don't know whether your history of bowel paralysis is likely to be relevant to them .

But if you are not taking any B12, then i would think they need to be discussed with a Doctor to put your mind at rest.

My medication I’m taking is by doctor vitamin b co strong is above - Each tablet contains the active substances: 20mg nicotinamide, 2mg pyridoxine hydrochloride, 2mg riboflavine and 4.85mg thiamine mononitrate.

The T3 results ranges are 4.1 to 6.8

I have had my liver investigated with ultrasound etc and didn’t show any fatty liver.

What I read that bone marrow issues can happen like blood cancers and could it be connected taking T3 would that speed everything up causing the problem. I Google too much z

The doctors are crap they signed off the test as normal . But they signed off the T3 at 2.1 for a year so they are crap.

I need a full blood count and crp check I think .

My AST is normal which suggests not liver so bone?

if your Doctor has seen theses results and said they are not concerned by the high B12 / ALT / Serum Alkaline phosphatase , then did you ask them to explain why they are not concerned ?

If 'yes', what did they say ?

If 'no' , go and ask them.

If you are concerned they show blood cancer , then discuss this with a Doctor and stop googling stuff you don't understand and couldn't do anything about without a doctor anyway.

It's a waste of your time, and is just stressing yourself out for no benefit (and you're already far too stressed by the problems you do have)

Your post is difficult to unscramble and we lack ref ranges but looking at the above labs your T4 to T3 conversion appears poor.....high FT4 with low FT3.

Poor conversion results in reduced FT3 level

This causes hypothyroidism/ under or wrong medication

TSH is far too high indicating hypothyroidism/ incorrect thyroid medication but....TSH is not an accurate marker.

Constipation, digestive issues and weight gain ( inc legs) are symptoms of hypothyroidism

The timing of your results is wrong....aim for close to 9am....everything tested at the same time.

Test only after at least 6 weeks on a stable dose to allow hormone to settle into the system

Only a full thyroid test, to include the following, will provide the info required to accurately analyse your thyroid function

TSH, FT4, FT3, vit D, vit B12, folate, ferritin and thyroid antibodies TPO and TG

Don't panic about high vit B12 at least not until you have had a recent test! It may heve been a blip.

Have you been tested for thyroid autoimmune disease / Hashimotos'....I have read ( link follows) that autoimmmune disease can raise vit B12

Stress levels and anxiety rise with hypo

Full test will reveal the problem but I guess you are possibly suffering from low T3

T3 is the active thyroid hormone and for good health must be available in a regular and constant supply...your lab is low

hypo/ low T3 causes all manner of symptoms ....medics often do not join up he dots!

Meanwhile

I am now on 30mcg T3 125mcg t4

How long have you been on this dose and has anything improved?

Post new labs, tested with correct timing, and those will point the way forward..members will help you to interpret them.

Think you may have misread the TSH ......in image TSH <0.01

sounds like the image is the 'latest' results unless Chouchou lets us know otherwise , so the high B12 may well be current, and sounds like B12 is not currently being taken . No idea if she was taking any B12 previously though.

hi

The only vitamin I took for B is vitamin cod strong and ingredients are above. Plus extra 100 of thiamine.

I don’t take a specific b12

I did notice 6 months ago my b12 was over just slightly so I stopped and started again a mo th before test.

I do have severe bone pain too.

When I take T3 must be cortisol as I get a high feeling straight away

The timing of your thyroid tests was "near enough as to makes no difference"

yes , we do say "24hrs from last Levo/ 12 hrs from last dose T3" . but 27 hrs Levo / 13 hrs T3 is near enough , so i don't seen any need to retest them . and i don't see what further information you will get if you retest at 24hrs / 12 hrs exactly ...... it will just cost you more money for no real purpose.

if this is your second over range B12 and you were not taking any B12 for either of them. then ask a Doctor who can see your full medical history if the over range B12 /ALT/ Serum alkaline phosphatase is a concern or not.

You can't just assume your cortisol is high because you feel 'high; Stick to information you know .

You're welcome.

Thyroid disease can be a nightmare but it can be resolved with the correct medication...so hold that thought!

It can also cause a huge list of debilitating symptoms which are helped by correct medication.

It's difficult to think straight and to stay focussed when you feel rubbish...but keep working on that. I understand, like many here, I've been there too.

One step at a time otherwise you will feel overwhelmed...

Start with a full thyroid test, correctly timed and after a steady dose for at least 6 weeks.

Post the labs and we will all try to help.

I wouldn't be writing this had I not had amazing support from here....hang on in there!

Is 30mcg. T3 a lot? They approved 25mcg so I increased it to 30mcg I’m very nervous I’ll get in trouble.

it depends on how much you need!!

30mcg T3 125mcg t4.....that would be a large dose for most people....but not all!

Are you having any symptoms of overmedication?

The signs and symptoms of thyroid hormone overmedication can vary but may include:

Elevated pulse and blood pressure

Anxiety, nervous energy, tremors

Feeling irritable, overemotional, erratic, or depressed

Difficulty concentrating

Difficulty sleeping

Fatigue or feeling extremely tired

Feeling overheated, even when others are cold

Diarrhea

Feeling like your heart is skipping a beat or racing

Weight loss with no change in your diet or exercise routine

Increase in food intake with no weight gain

In some cases, the symptoms and side effects of taking too much levothyroxine are similar to those of hypothyroidism. For example, you may feel more exhausted than usual, feel achy (as if you have the flu), or feel jittery and anxious. You may also gain weight.

It's not unheard of for a person to go to their medic because they think they need a higher dose of their medication only to find out their symptoms are actually being caused by thyroid overmedication.

What difference did increasing to 30mcg make?

With T3 you cannot chop and change doses....the supply needs to be constant.

So was the test in the image above done when you were taking 30mcg T3 and 125mcg T4 ?

fT3 was 4.4 [4 -6.8] .....last dose 13hrs.

fT4 was 20.6 [12-22] .... last dose 27 hrs.

TSH <0.01

if so , had you been taking that dose for 6 weeks before the test ?

if not ,what were you taking for 6 weeks before that test ?

yes that was the dose of 30mcg and 125mcg I was on it for 2 and half weeks I think. I was in 25mcg before

Looking at the list -

no elevated pulse no blood pressure

I am extremely tired but I am not sleeping great

Constipation still

Why do I get so insanely tired after thyroxine ??

I am only 49 kilos

No Elevated pulse and blood pressure - also had ecg all fine

Anxiety , nervous energy, tremors - NOT

I’m nervous because I can’t get it right and not thriving

irritable, overemotional, erratic - no

Difficult sleeping yes but most of it because I can’t digest .

feeling extremely tired yes - but also not digesting food

I’m freezing cold but definitely not as bad as before I used to be crying in bed I was so cold with 5 layers

Zero stomach motility or limited and having weekly colonics still

Hypothetically could I have that enzyme thing blocking the T3? Cortisol?

I have lost muscle a lot but gained fat .

The only reliable test I have when not on any medication at all was 2018 and tsh 5.7 and t4 14

I was in hospital with impacting of stools and they still didn’t give me thyroxine infact they told me it was normal. I started self medication for years which maybe has screwed me up completely

but i know you've already been told that to test thyroid hormones properly you need to be on an unchanged dose for 6 weeks .

,, but instead of doing that , you decided to increase your dose from 25mcg T3 to 30mcg .. and then give us some test results from only 2 and a half weeks .

if you keep doing the opposite of what you are advised then you will probably carry on being in a mess... no one can help you much if you ignore most of what they say .and straight away do the opposite .

Stop fiddling around with your dose..... sounds like you already know that taking increasing amounts of thyroid hormone without testing properly , is what got you into this mess in the first place.

" You are what you eat..." we literally are ....... so it's pointless asking technical questions about "why do i feel so bad ? is my T3 this /that or the other " .... of course you will feel horrible while your digestion is this messed up .. you must be pretty malnourished for a start .....and if your bowel is as compromised as it sounds from things you've said so far, and if you are vomiting regularly just to get by .. then you will obviously not feel well , even with perfect thyroid hormones .

the right dose of thyroid hormones might (very gradually ,over long time) be able to improve you bowel function IF it can be improved .. but none of us on here know what is going on with your bowel history because you have never told anybody here what that medical history is.

If you get you bowel working better , then later on , the rest of your body will probably function a bit better,

But if you want thyroid hormones to help your bowel , then you have to follow the rules that allow you to find the right dose ....and then stick to it .

But so far you haven't been willing to accept that there are rules to finding the right dose ... and until you do , and follow instructions ,and give the necessary information when asked for it .... so that people here can help you find the best dose .... then all your other questions about thyroid hormones are a bit pointless.

Sorry to be blunt , but it's what you need to do if you want any chance of finding what your dose should be .

ok agreed. You are right. Sorry to bother you. I am just alone and scared and need to stop fiddling. Thank you 🙏 sorry to bother you and thank you so much for the time and detailed answers. They did full blood count and crp today with electrolytes. Ofcourse nervous until tomorrow. The doctor sent message as I asked by email that I am concerned re b12 and his answer was stop all vitamins. This is a nightmare for me as I know I don’t absorb enough through diet. Thank you
