Hope your all feeling well, I have a question that I cannot find any answers to, so I thought I would try here :)

I have recently discovered a small lump on my thyroid. It’s tiny, millimetres big, and I had an ultrasound on my neck a couple months ago (that was for my lymph nodes, turns out I am recovering from an infection that’s taking ages to clear up), anyway, I wasn’t aware of this when I had my scan, but blood tests and everything have been taken and it’s all coming back normal. Everything is functioning normally, no major weight gain/loss etc. and I wanted to know what this is.

I have jaw pain randomly throughout the day, the front of my lower neck hurts from time to time, but the lump is not growing as far as I can tell. I have been trying to monitor it for a couple weeks and there are no noticeable changes in size and is located in the centre of my thyroid. It can be moved around but will return to its normal spot. It could be swollen glands as lymph nodes between my jaw and ears are swollen but I wanted to put it to you guys to see if anyone can provide any info.

I also have a vitamin b12 deficiency but am not vegan or vegetarian if that turns out to be useful info for anyone. Also, 18M with some distant family with thyroid function issues. If anyone has had a similar experience, it would be very nice to hear from you :)

Hi benjago and welcome to the TUK forum.

Could you share the actual results for the thyroid tests that have been carried out, as even though medics have said these were ‘normal’ (ie within range), these could still indicate if your thyroid is struggling. With any test results you share, we need to also know the ranges (in brackets after the results), as these can vary between laboratories.

The usual tests may include:

TSH

FT4

FT3

Plus antibody tests

I understand that you have low B12 and have received advice on the PA forum, but have you also tested other key vitamins that support thyroid health, ie folate, vitamin D and ferritin?

What have you been prescribed to address your B12 deficiency?

Hypothyroidism is often revealed once B12 injections have started

Did you test positive for pernicious anaemia?

Also you had coeliac blood test done. Was this negative?

You need to retest thyroid, including thyroid antibodies, and vitamin D, folate and ferritin

 

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested.

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies 

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. 

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis 

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended on here that all thyroid blood tests early morning, ideally just before 9am and last dose levothyroxine 24 hours before test 

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

if GP can’t/won’t do full test
