Folate 16.3 ug/L 1.9-25

t4 22.2 (11-22)

Tsh 3.36 (.27-4.20)

Did stop the multivitamin two days before for the folate, folate and b12 have jumped up!

Still feel like crap, anxious, fidgety, higher heart rate too, maybe it’s just stress and anxiety?

Was test done early morning

No Ft3

No thyroid antibodies

No vitamin D

No iron or ferritin

TSH is relatively high

(suggesting possibly hypothyroid/Hashimoto’s)

Hashimoto’s frequently starts with transient hyperthyroid results and symptoms before becoming increasingly hypothyroid

So next step you need FULL Thyroid and vitamin testing done

you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested.

Very important to test vitamin D and ferritin

If ferritin is under 50…..full iron panel test for anaemia

What’s your diet like

Are you vegetarian or vegan

Gluten free or dairy free

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies 

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. 

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis 

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended on here that all thyroid blood tests early morning, ideally just before 9am

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

Vitamin d and ferretin wee done last month and fine so we’re the antibodies. Not sure we’re to got from here? Maybe stress and anxiety is make my heart rate higher?

NHS won’t test TG antibodies unless TPO antibodies are high/abnormal

As your TPO antibodies are negative/normal you will need to test privately

And test TSH, Ft4 and Ft3 at same time

Test early morning

Vitamin D and Ferritin good

B12 and folate were very poor

Now improving

Multivitamins never recommended on here

Most contain iodine, which can exacerbate thyroid disease

How long have you been taking multivitamin?

Which multivitamin have you been taking

Suggest you stop and just take

Vitamin B complex

Vitamin D

20% of Hashimoto’s patients never have high thyroid antibodies

Ultrasound scan of thyroid may get diagnosis if both TG antibodies and TPO antibodies are negative

Also possible to have a hot nodule

This is where nodule makes thyroid hormones regardless of TSH…..but you would expect low TSH….and high Ft4

Strongly recommend stopping this

Contains iron, you don’t need

Contains iodine, which can significantly exacerbate thyroid issues

Instead to improve low B12 and folate….supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) 

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules.

(You can tip powder out if can’t swallow capsule) 

Thorne currently difficult to find at reasonable price, should be around £20 

If you want to try a different brand in the meantime, one with virtually identical doses of the ingredients, and bioavailable too, then take a look at Vitablossom Liposomal B Complex. Amazon sometimes has it branded Vitablossom but it's also available there branded as Yipmai, it's the same supplement

it’s only a low dose of iodine though only been taking it for 2 months as the thyroid up and down since 2018 surely that won’t effect it that much?

140 micrograms - which the label says is 90% of daily requirement.

150 micrograms a day is often suggested as our daily requirement.

So, whilst some people consume much more, I wouldn't be too ready to dismiss it as being a low dose.

to be honest I’m confused didn’t think they would cause too much trouble as I’ve had this problem for years and only been taken them for 2 months stopped for two days haven’t notice a difference

You TSH is (relatively) high

But in these circumstances you would normally expect Ft4 to be low or low/normal

Your Ft4 is borderline high

GP most likely won’t think it’s anything other than pretty normal

Retest thyroid levels 6-8 weeks after stopping multivitamin. Iodine supplement could be causing significant issue

what about the heart rate? Still sits at 70-80 maybe 65-79 when sitting or laying when it use to be 50-60 sitting or laying (awake) it does go down to 48-68 at night when I’m asleep

so next steps

A) stop multivitamins

B) take good quality vitamin B complex instead. Remember to stop this 5-7 days before retest

In 6 weeks after stopping multivitamins and starting vitamin B complex

C) NHS won’t test TG antibodies unless TPO antibodies are high/abnormal 

As your TPO antibodies are negative/normal you will need to test privately 

And test TSH, Ft4 and Ft3 at same time. Test early Monday or Tuesday morning, ideally just before 9am

you could do with checking your T3. With your TSH being quite high and also your T4 high you might not be converting T4 to T3 very well hence the high TSH and not feeling like you should. My TSH was high, T4 high but T3 was on the floor. I did the DIO2 gene test and I found out I have this faulty gene. As soon as I started to take T3 liothyronine medication alongside T4 levothyroxine I felt 100% better.
