Symptoms but results within ‘normal limits’

hello,

Asking for some advice re my recent bloods.

I have a range of symptoms which have been ongoing for a number of years, but have become more acute since stopping the pill. These include hair loss, very cold hands and feet, tiredness, some weight gain despite normal activity, swollen tongue and now very light and frequent periods as well as dry skin, but also occasionally tremors.

my recent results are as follows:

T4 83 (59-154)

TSH 2.98 (0.27-4.2)

Free T4 13.6 (12 - 22)

Free T3 4.9 (3.1 -6.8)

Thyroglobulin 25.6 (0-115)

Thyroid peroxides antibodies <9 (0-34)

Vit D 51 (50-200 normal range)

b12 428 (197-771)

Folate 7.7 (>2.9)

ferritin 49 (13-150)

I have been taking vit d supplements recently and started a couple of days prior to the test.

I’ve read many posts but hard to unpick as someone unfamiliar whether or not there is something here for me to push further on (apart from vit d which does seem on the low side?).

any advice would be much appreciated.

welcome to the forum

Approx how old are you?

Pre menopause?

Folate, B12, ferritin and vitamin D all on low side

Suggest you work on improving and then retest in 3-4 months

Optimal vitamin levels are

Vitamin D at least around 80nmol minimum and nearer 100nmol likely better

How much vitamin D are you currently taking

Folate and ferritin at least half way through range

B12 at least over 500

What’s your diet like

Vegetarian or vegan, gluten free or dairy free

Low ferritin

Look at increasing iron rich foods in diet. Red meat daily, liver or liver pate once week etc

Only add one vitamin supplement at a time

Waiting at least 2 weeks before adding another

Starting with vitamin D

Then B12 ……then separate vitamin B complex

thanks for the reply - very much appreciated.

I’m 38 so would be early / peri menopause. It’s not out of the question I suppose.

I’m taking vit D3 25mg at the moment. I’m not a vegetarian though I do try and limit my meat consumption during the week…. No other dietary limits.

25mg vitamin D is only 1000iu daily

Likely to need at least 2000iu and more likely 3000iu daily for at least 3 months to increase vitamin D to at least over 80nmol

NHS Guidelines on dose vitamin D required

ouh.nhs.uk/osteoporosis/use...

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

leedsformulary.nhs.uk/docs/...

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

mm.wirral.nhs.uk/document_u...

But improving to around 80nmol or 100nmol by self supplementing may be better

pubmed.ncbi.nlm.nih.gov/218...

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Test twice yearly via NHS private testing service when supplementing

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function.

There’s a version made that also contains vitamin K2 Mk7.

One spray = 1000iu

Low B12 symptoms

b12deficiency.info/signs-an...

methyl-life.com/blogs/defic...

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months.

once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

low folate

At least a week after starting on B12

Consider supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

Thorne currently difficult to buy

If you want to try a different brand in the meantime, one with virtually identical doses of the ingredients, and bioavailable too, then take a look at Vitablossom Liposomal B Complex. Amazon sometimes has it branded Vitablossom but it's also available there branded as Yipmai, it's the same supplement

amazon.co.uk/Yipmai-Liposom...

or available as Vitablossom brand here

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

endocrinenews.endocrine.org...

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate methyl folate supplement and continue separate B12

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Never supplement iron without doing full iron panel test for anaemia first and retest 3-4 times a year if self supplementing. It’s possible to have low ferritin but high iron
