Please any advice. I have Hashimoto's and insulin resistance, I always have low TSH but my T3 is always low and T4 in range, I came off T3 in May myself due to concerns of osteo, it turns out I do have osteoporosis in my back really bad, please see attached results, I am concerned there is a  pituitary gland problem or Parathyroid.

Was test done early morning, ideally before 9am and last dose levothyroxine 24 hours before test

Do you always get same brand levothyroxine at each prescription

If yes, which brand

How long since you stopped T3

When were vitamin D, folate, ferritin and B12 last tested

What vitamin supplements are you currently taking

Vitamin D?

Magnesium?

K2 MK7?

Do you have autoimmune thyroid disease, aka Hashimoto’s and are you on strictly gluten free diet

Are you on any other medications, eg Metaformin

Osteoporosis can be linked to LOW Ft3

No not diabetic, what does biotin do please. Yes Hasimotos but no matter how low my T3 and T4 go I still have TSH 0.01

Biotin is used in many lab test machines..if taking supplements that contain biotin it can falsely affect test results

Exactly what vitamin supplements are you currently taking

Many Hashimoto’s patients have sluggish or non responsive TSH

Most important results are ALWAYS Ft3 followed by Ft4

Your results suggest you are under medicated

Next steps

Always get same brand levothyroxine at each prescription

Try splitting your levothyroxine dose into two smaller doses

Get vitamins tested and come back with new post once you get results

Try strictly gluten free diet/dairy free diet

My consultant is ordering pituitary tests. He says osteo cause mainly low TSH but this now won't come up. Not taking biotin. I have never converted T4-T3 well. Been off T 3 for 3 months. Vitamins are all good. I am totally gluten Dairy Free. I have B12 injection every 8 weeks this helps, the diet is perfect as can be. Just rapid bone loss that my consultant said aging I am not so sure. On HRT do resistance training and walk miles and run. I am insulin resistant I manage it with a strict diet and excise.

Exactly what vitamin supplements are you currently taking

You’re going to need addition of T3 reinstated

B12 hydroxocobalamin B complex, Vit D 400 but I take more 800 Vit K2 Aristo Levo I convert this better and no lactose. Magnesium and my diet is full of all the nutrients as well, Folate is top range.

What are your most recent vitamin D results

Are you taking vitamin D with vitamin K2 mk7

Which magnesium

With Hashimoto’s, it’s not how good our diet is…..but how well or badly we absorb nutrients

Your Ft3 is too low…..on replacement thyroid hormones it’s low Ft3 (not low TSH) that’s implicated in osteoporosis

Looking at previous posts you have Hashimoto’s

So it’s essential to test vitamins at least once year

Always test thyroid levels early Monday or Tuesday morning, ideally before 9am and last dose levothyroxine 24 hours before test

Don’t test during heatwave

Stop any supplements that contain biotin a week before blood test

Make sure that before taking any osteoporosis drugs your doctor checks out your parathyroid gland because if your parathyroid gland (not the same as your thyroid gland - they just happen to be tucked in behind the thyroid gland) If they are causing your osteoporosis you need to have that sorted out as no amount of osteoporosis drugs will fix the problem.

What was your FT3 level when you stopped taking T3? Just taking T3 does not cause osteoporosis. Long term very high levels of T3 cause osteoprosis.

However, one thing doctors never seem to grasp is that you need good levels of T3 to prevent osteoporosis. And your level is not good. In fact, I'm surprised you're able to get out of bed!

Low TSH does not cause osteoporosis, either. TSH has nothing to do with it.

How are your nutrient levels? I take it you have had your calcium tested, so, what was that result? How about vit D? Vit D and magnesium are far more important to bones that calcium in an adult.
