It was only a month ago you were starting on just 50mcg levothyroxine

It takes 6-8 weeks for each dose increase in levothyroxine to have full effect

Typically dose levothyroxine is increased slowly upwards in 25mcg steps until on approx 1.6mcg levothyroxine per kilo per day

Approx how much do you weigh in kilo?

guidelines on dose levothyroxine by weight

Even if we frequently don’t start on full replacement dose, most people need to increase levothyroxine dose slowly upwards in 25mcg steps (retesting 6-8 weeks after each increase) until eventually on, or near full replacement dose

NICE guidelines on full replacement dose

nice.org.uk/guidance/ng145/...

1.3.6

Consider starting levothyroxine at a dosage of 1.6 micrograms per kilogram of body weight per day (rounded to the nearest 25 micrograms) for adults under 65 with primary hypothyroidism and no history of cardiovascular disease.

Traditionally we have tended to start patients on a low dose of levothyroxine and titrate it up over a period of months. RCT evidence suggests that for the majority of patients this is not necessary and may waste resources.

For patients aged >60y or with ischaemic heart disease, start levothyroxine at 25–50μg daily and titrate up every 3 to 6 weeks as tolerated.

For ALL other patients start at full replacement dose. For most this will equate to 1.6 μg/kg/day (approximately 100μg for a 60kg woman and 125μg for a 75kg man).

If you are starting treatment for subclinical hypothyroidism, this article advises starting at a dose close to the full treatment dose on the basis that it is difficult to assess symptom response unless a therapeutic dose has been trialled.

BMJ also clear on dose required

Bloods should be retested 6-8 weeks after each dose change or brand change in levothyroxine

Hi again SlowDragon, thanks for replying to my post :)

I decided to quit taking the T4 about 2 weeks ago as my symptoms was worsening and decided after reading about the T3 Circadian rhytm protocol to try it out as i also have problem with very low cortisol in the AM and afternoon.... I have so far felt no ill effect and actually feel more alert\higher energy and also much reduced bodily aches compared to when i was using T4 only or T4+T3.

I am currently only on 12.5mg T3 which i take around 4am when i wake up to pee, i will be trying to increase every 2 weeks by 6.25mg as long as my stock last which is not long so i hope someone can help me atleast find a replacement of some sorts :) i will also try to get my GP on board but i fear i will be shut down as usual but one can only hope!

It’s extremely common to feel worse before you start improving

You won’t get T3 from a GP, they are unlikely to have even heard of it

There are a few endocrinologist prescribe T3 alongside levothyroxine. Virtually none that will allow just T3, and most would always start with levothyroxine
