Raised morning and lunchtime cortisol

I recently had my results from Medichecks of the saliva test . I have hashimotos and currently under medicated and deficiency in vitamin D all being addressed with increase of medication. However my GP did not understand the Cortisol results and is seeking advice from an endocrinologist that was a week ago. I feel dreadful on waking no energy very anxious and hot sleeping very poorly. At about 4 pm I pick up with some better energy levels which continues until the next morning. This has been happening since March . I am wondering if this could be because of my Hashimotos ? I would appreciate any advice or has anyone had a similar experience ?

Results Adrenal Hormones

Cortisol waking 42 .100nml/L

Range ( 6-21)

6.0-21.0 possible Addison's

Cortisol 12 noon 12.100nml/L (Range 1.5-7.6)

Cortisol 4pm 3.210 nml/L (Range o-5.5)

Cortisol before bed 1.5 nml/L

Range(0-2.05)

5.0 possible Cushing's

(Range 0-2)

Looking at previous posts you are, as you know, under medicated for thyroid….after dose was inappropriately reduced from 175mcg levothyroxine to 75mcg

Clearly you are now very under medicated

Ferritin 51.50 ug/L ( Range 13-150)

Folate Serum 6.29ug/L (Range 3.89)

Vitamin B12- Active 71.000pmo/L (Range 37.5-188)

Vitamin D 32.90nmol/L (Range 50-200)

TSH 6.86 mU/L ( Range 0.27 -4.2)

Free T3. 3.92 pmo/L ( Range 3.1 6.8)

Free Thyroxine 14.000 pmo/L (Range 12-22)

Thyroglobulin Antibodies 20.400 IU/mL

(Range< 115)

Thyroid Peroxidase Antibodies 600 IUmL

Adrenal glands/cortisol tries to compensate for lack of thyroid hormones. Typically cortisol rises initially when under medicated for thyroid ……if under medicated for long time, eventually adrenals can’t keep up and cortisol drops low

It can take many weeks/months for cortisol to slowly drop, even after levothyroxine dose is increased

Have you now had dose increase in levothyroxine to 100mcg

Bloods should be retested 6-8 weeks after any dose change or brand change in levothyroxine

Might need further increase in levothyroxine after next test

Meanwhile working on improving low vitamin levels is essential too

Exactly what vitamin supplements are you currently taking

Are you now on strictly gluten free diet

Thank you for this. I have been on increased f dose for 10 days now on 100mcg will retest in 8 weeks. I have started taking Vit D and K2 and ordered Thorne B 12 complex. I have been fully gluten free for 3 months now. I have been feeling worse since medication increase but I understand that can be expected for a few weeks. I had the flu jab last week and have felt very tired even more than usual and very cold .
