Seeing Endo tomorrow, need advice following results

Hi, I previously posted my blood results following a reduction in my T4, I am seeing my Endo tomorrow having waited so long and feeling so horrendous I increased my T4 myself back up to 100 instead of the 75. I have just had my results from blood tests I had done last Friday they are Serum TSH <0.05 again this says I am over replaced, Serum free T4 level 12.1 pmol/L (normal) and T3 level 4.5 pmol/L (normal)

The ranges they give as ok are TSH 0.30-4.20 min/L, T4 12.00-22.00 pmol/L, T3 4.00-6.80 pmol/L

Even I can see that my results are only just within the ok scale, I’m fed up of feeling rubbish, I am always tired and have put on weight again, please can you advise me as to what to say to my Endo

First you are NOT NOT NOT over-replaced!

TSH may well be suppressed but your thyroid hormones say you are VERY under-medicated for someone on levo. Free T4 is 1% along the range; free T3 is better but clearly still far too low.

You need more levo. You want free T4 to be 19 or higher on this range - and free T3 more than half-way too. The only good in these results is that free T3 is higher than free T4 - which is really really low! Once both free are nice and high it should be possible to lose weight - but it's probably totally impossible at the moment with such low thyroid hormones :(

Are you still also taking T3 ?

I am taking 100 of Levothyroxine and 20 of Liothyronine. I followed your advice about blood tests.

My results are: Serum free T3 level = 4.4 pmol/L

Serum free T4 level = 11.5 pmol/L

Serum TSH level = <0.05 min/L

On almost any dose of T3 TSH is almost always suppressed

Many people on levothyroxine plus T3 need both Ft4 and Ft3 roughly 50-60% through range

Clearly Ft4 is currently extremely low

Bloods should be retested 6-8 weeks after any dose change or brand change in levothyroxine or T3

How long have you been on 100mcg levothyroxine

Do you always get same brand levothyroxine at each prescription

When were vitamin D, folate, ferritin and B12 last tested

What vitamin supplements are you currently taking

Hi I am taking 20 of T3 with the 100 T4. I increased it over 6 weeks ago. All my previous test results with these doses have been much the same as these results. My previous results have shown both my iron and vitamin D to be low. I have been taking a good vitamin supplement but they didn’t test these this time.

Hi, can I ask you about your supplements please? Are you taking a good multivitamin, or taking separate supplements?

I am not surprised you are feeling ill.

Have you tried going gluten free? I was tested for Cealiac which was negative, but I do feel better on gluten free, so Instick to it.

It can be worth eliminating things which might be causing you trouble. I tried stopping all dairy but I didn't really see any improvement in my health. So I use ghee in cooking (and cheese very occasionally) but I cut out cow's milk and use goat's milk, which you can get in most supermarkets now and I feel OK on that. Just trying anything I can really.

I eat low carb too, but not high fat. I can't lose an ounce if I eat high fat unfortunately, though I feel OK with it. Good luck.

Well then that explains the low FT4 and low TSH. You did not mention the 20mcg lio.
