Thyroidectomy and on Levothyroxein 100mcg for 3 weeks. Test Result TSH 0.11 mIU/ L Low FT4 30.1 pmol/ L High. Any advice ? Thanks

I have been on Levothyroxein now for 3 weeks following a total thyroidectomy due to cancer.

I have just received my thyroid test results as follows

Thyroid stimulating hormone0.11 mIU/ L 0.27 - 4.20 mIU/L Low

Free T430.1 pmol/ L 12.0 - 22.0 pmol/L High

I am seeing my consultant on 26th July. I am feeling very anxious about these results. Any advice would be greatly appreciated, with many thanks in advance for any support.

With those results it might be an idea to reduce your Levo a little to bring FT4 into range. However, as you have had a thyroidectomy it's almost a necessity to see your FT3 level too so next test ask for TSH, FT4 and FT3 to be tested together to give a full picture. You need to see how well your body converts T4 to T3, you may need a Levo plus T3 combination rather than just Levo alone.

As a vegan what vitamin supplements are you currently taking

When were vitamin D, folate, ferritin and B12 last tested

Likely to need to supplement to maintain optimal vitamin levels

Is your GP and endocrinologist aware you’re vegan

On levothyroxine we must have GOOD vitamin levels for levothyroxine to convert from Ft4 to Ft3

For full Thyroid evaluation you need TSH, FT4 and FT3 tested.

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Is this how you did your tests?

It is most unlikely that you will ever feel well on levo alone, perhaps not even on combined levo/liothyronine. Do not delay too long if either of these fails. Your only choice will then be NDT, not available on the NHS except perhaps by Private prescription.Don't be afraid to ask questions of your GP or endo, they tend to say nothing if you don't ask.

Easier to get T3 prescribed alongside levothyroxine than NDT.

Before considering adding T3 all four vitamins need to be tested and optimal levels
