Hi, I'm Maggie from Macedonia. I was diagnosed with Hashimoto 20 years ago. Since then, my TSH results have never been good. I take Euthyrox Levothyroxine Sodium tablets of 100 to 150 micrograms, depending on my recent results. I am 160 cm tall and weigh 56 kg. I also take selenium 100 mg / day. All these years my TSH has never been below 4 mIU / ml exept one time in the begining, when it was below 0.01mIU / ml. Should I change my therapy?

To answer your question: almost certainly
Once on T4 meds, you should increase dose by 25 mcg a day every 6 - 8 weeks until TSH is less than 2 (probably less than 1) and you feel well. But just testing TSH is not nearly enough - you also need to test your actual thyroid hormones - free T4 and free T3 - to see how they are doing too. [TSH is a message from the pituitary telling your thyroid to work harder if it doesn't function as it should, so a high TSH usually means not enough thyroid hormone is produced; but you need to measure your thyroid hormones too because sometimes your thyroid doesn't receive the message from the pituitary or deal with it properly]. These should both be in the top third of the reference range.
And ideally also test key nutrients - ferritin, folate, vitamin D and B12 - because your meds work best when these are good and because for so many of us, especially with Hashis, they're not.

My results for free T4 are always in the the reference range. But I was never tested for free T3.
I'd really try and get more testing done, then, if I were you, to include free T3 and the key nutrients I mentioned. And just being in the reference range for free T4 isn't enough - you are likely to need to be in the top third of the range, maybe the top 25%, to feel properly well - languishing in the bottom part of the range is no good at all x
