Very low cortisol levels. Need help desperately

Reposting... I can't seem to see my previous post.

I'm on 30mcg of t3 and 50mg of thyrovanz.

Can I get some help,advice and suggestions on my results please. I'm struggling so much even to do the basis of tasks.. I cant really get out of bed. I'm shattered by 4pm . Spoke to my gp who completely dismissed it of course. The gp ordered a morning cortisol blood test which was

272 (133-537) . Didnt do anything with the results either.

I've tried nutri adrenal x2 before not had much luck..

I would really appreciate all your help.

Have you spoken to your GP about the results? Rather than taking supplements to try to improve it you need to have some further investigations to find out what is going on, especially with the symptoms you are experiencing.

Gp completely dismissed it. Saliva test is not recognized by the NHS unfortunately.

Your cortisol level is fairly low so should be further investigated. If you are on the oestrogen contraceptive pill or HRT or taking any supplements containing Biotin then that may give you an elevated results. Perhaps you could show your GP the NICE guidelines which say you should have further testing. cks.nice.org.uk/topics/addi...

Hi Pauline - does this mean that when you stop hrt your cortisol levels drop? And if so, do they ever recover?

No, if on HRT the cortisol result will give an elevated result due to the HRT, to get an accurate result for cortisol you need to be off it for 6 weeks prior to the test to get your normal cortisol level.

That would probably explain why my cortisol plummeted when I stopped hrt.

I'm on b complex.. will that elevate results, I wonder? Plus cant sleep at night.. if i do its broken sleep all throughout 😒

I've never seen it suggested that it does affect cortisol.

Hi Maya - my GP completely dismissed mine too, eventually relented when I pointed out DHEA level and emailed the test to an nhs endo who said I should expect these levels at my age. I’ve since changed surgeries and hoping for more help. Good luck with yours.

My sister has high cortisol and often has a saliva test done by her GP

I have recent cortisol results very similar to yours. My NHS cortisol was also very similar to yours. I wrote to my GP with a copy of my results with a list of my major symptoms and asked for a referral to an endo. She agreed and the endo has arranged for me to have a short synacthen test. This is the test you need.

Oh thanks..

I an endo appt end of February... but how can I convince them to test me for a short synacthen test? I know they are very strict with the 'in range ' results.

Look at the symptoms list for low cortisol and write down those which you suffer from. Have you had your sodium and potassium tested? Keep a diary of symptoms to show your endo.

Does potassium and sodium test need to be prepared for ? Ie: fasting ? First thing in the morning? And meds we need to stop before hand?

The range covers the whole 24 hours but cortisol varies through the day being at it's highest level first thing in the morning which is why it is important to do a morning test. Be aware that not all Endo's are conversant with pituitary/adrenal issues as most of the deal with diabetes.

You were lucky with your GP, when I sent her my cortisol results, she forwarded them to an nhs endo who wrote back saying this should be expected by a woman of my age. I then had a consult with private endo who asked for further tests and GP told me she wouldn’t do them. I’ve just switched to a different practice and hoping for more co-operation.

The GP I now see in my practice is very good and will do what tests she can. I have intracranial hypertension a rare brain disease which GPs don't know much about so she is happy to refer because of my symptoms.

You could try going private fir the short Synacthen test. I will forward you details if my private only Endocrinologist.

I would go private but, tbh, I’ve already spent enough on a private endo whose requests were ignored by gp and my business has been decimated by Covid with absolutely no government Covid support. I think it’s time the system I’ve been paying into for 40 years stepped up to help.

Your cortisol test falls within range so your GP wont do anything. My cortisol was tested at 107 (150-550) and the endocrinologist dismissed it despite way under, so I doubt they will investigate cortisol within range. Mine dropped further to 95 and I was given some hydrocortisone 10mg, a new endo who is very good at thyroid but not adrenal stuff, he also couldnt find me a specialist to refer me to.I never use to sleep through the night but once I was taking my thyroid meds at night not morning I slept. I just eat well, regular sleep routine but mostly try and keep stress to a minimum, I do not handle stress at all since my thyroid was removed, my anxiety can wake me up (since TT) and this has been a bad year for me anxiety wise.

There is very little you can do about very low cortisol, I have tried most things, adrenal cocktail, supplements, NAX CT3M, natural progesterone, oestrogen etc nothing makes much difference and I weaned off the steroids as its not an answer just further weakens natural production. I am hoping to be referred to a bio identical hormone specialist if there is one and cover it as a whole. I am post menopause so obviously all my hormones are low.

The SS test showed my cortisol reaction is sharp, it went up to 650 in 30 mins so i have secondary hypo adrenal problem. In my experience GP's and endos know very little about thyroid and even less about cortisol.

Did you self medicate, as your doctor dismissed it? I really don't know what my next step is as my doctor won't support me with results as it's in range!!
